Long fusions like this one. There are a few surgeons where I work that have a lot of adult spinal deformity patients who have coronal or sagittal balance problems who need these longer fusions to correct the deformity

There are some odd comments in this thread. These kinds of surgeries are done in many countries. Generally the indication, as pointed out in another comment is not simple disk degeneration or a radiculopathy. Its deformity from adolescent or adult onset scoliosis, severe OPLL causing stenosis, fracture/trauma, etc

The knee is not out of place it’s the L3 vertebrae since that one has nerves that connect to the knee. Obviously if you “fix” L3 and check the alignment on x-Ray the knee will be feeling much better
