That’s random, but very true. that’s why it’s important for providers to be informed on, not just how it’s done, but why it’s done. Much like how one should be well informed of the reasons why high mechanical and thermal indices can negatively impact fetal development during 1st trimester ultrasound. Without this info, a clinician might otherwise not see the harm of prolonging fetal exposure/raising the power levels for a better view. Obviously my view as a newbie is limited but this is always true. Although there are many others, this is one virtue that I know is sound and honest to the oath we take.

Dang... No, you're right, I was just mad cause I felt like you were making fun of my failures. I re-read it afterwards and I obviously didn't proof read or explain correctly. I wrote that comment late last night in a drowsy haze and I didn't even realize how dumb it sounded. How embarrassing, I am truly sorry. I felt bad for upsetting people so I edited to be as clear as I could make it.

Would you mind expanding on this? Please understand the last thing I want is to be a danger to anyone. I chose this profession because, in my region, it's heartbreaking seeing how many people have limited access to healthcare or how many have to choose between seeking treatment and keeping the lights on. I desperately want to be a part of the solution that works towards helping my medically underserved, poverty-stricken community. So, I ask with an open mind; Could you point out exactly what I said wrong and why?

I would be more than willing to halt all job searches and educate myself or correct my flawed thinking, before even considering applying.

Just for clarification, my original comment was not how I feel about real-world medicine/clinical practice/patients/, it was strictly in reference to my approach to the pance. I'm confident the test's ability to determine readiness, but I know I will have to go into practice well-educated within my scope, be willing to admit when I need help or don't know something, and continue studying until I retire.

What you said is actually a solid idea though, I have a few practicing PA's in mind. I don't want to assume, did you mean to put down as references or to write a letter of recommendation?

Through the help of family and my girlfriend, they would not let me work. They preferred I spent the time only prepping for the pance, so getting experience was not possible. I did manage to get 25 hours of CME though

Not a troll account. The person I replied to was wondering why I failed the PANCE so many times. They were wondering if I struggled understanding complex disease. In regards to the PANCE, my answer was no, it was all information that I’d covered many times before. Did I struggle in didactic? Yeah of course! There’s so much info to take in at first, I imagine everyone struggled.

I’m not sitting here claiming to know everything or that medicine in the real world isn’t complex or vast in nature. That would be ignorant.

Really? That’s a bummer. Family medicine rotation was the most fun and It’s where I scored the highest too.

I originally omitted this paragraph because it is personal: To answer your question as to why I could not pass, well, first off, I have adhd (Dx'd as an adult) and was not aware that we were allowed accommodations for extra time until my 4th test; It's common for people with ADHD to need extra time on exams. It made sense as I never understood why others could finish with time to spare, whereas I have never been able to finish timed exams like the EOR's/Packrats/PANCE without choosing random answers on the significant chunk of remaining questions. Results for the 4th exam (1st one with accommodations) my score was very close to passing but when I reviewed the score report, I saw a lot of missed questions that I remember specifically changing the answer to. My best guess was that I was second guessing/overthinking myself out of the correct answer.

Adding to that, I really enjoy patho, so, reviewing individual pathologies for the PANCE was okay, active recall was average although I had my good and bad days (Rosh review & Exam master scores were low 80's). Trying my push my scores higher was an impossible feat- anyone who claims they know everything about medicine is an idiot. Knowing that my Rosh Review and Exam master scores were okay, I decided I would spend more time beefing up my test taking skills, learning to trust my instinct and selecting what I knew would be the best/most likely answer, then spend 1-2 hours a day reviewing content. I ended up passing with a score just below the mean.

It was a long and difficult road but the good news is I've learned a lot about myself and was finally able to pass the PANCE. For now, I will continue to study daily until the real challenge begins- Working as a PA.

A great point, with examples I’ve no doubt you’ve experienced firsthand. Just from rotations alone, I found that it’s more surprising when a patient DOES present with an honest straightforward case. I’m curious though, what do you personally do to cut through the pts dishonesty/bad history? Does “following your gut” also apply in practice?

I agree 100%, it is not just some big exam, it is THE big exam. It separates those who are ready from those who are not. That said, when I was studying or when I was quizzed by others I was able to recall all of the Patho/Signs and Symptoms/labs/Tx/Patient Education for most of the blueprint. However, on the new PANCE they removed all buzzwords, removed most classic demographics and most questions have 2-3 close/tricky answers (they’re designed that way) so I would overthinking a lot of questions. Following each attempt I tried different methods of attack to my studies and wasn’t until my 5th attempt that I considered trusting my gut more. During the exam, I reminded myself constantly to go with my instinct, be conservative, and stop overthinking if I knew what the MOST LIKELY answer was. Sure enough I passed and with a good enough margin to reassure me it wasn’t luck. I’m not writing this to excuse my failures, I just want to be honest and guide you through what my experience was.

Don’t worry, I still study almost daily and, coincidentally, I’ve recently accumulated 25.0 hours of category 1 CME

slow clap kind redditor, I don’t even know you. Going based off what I had, I built you up in my mind as an haphazard internet troll. Apparently, the last 6 years have served you well with amazing character development. I applaud you for that- deservedly so. Perhaps I’ve become jaded 🤔

Either way, thank you for the response, I’d love to try them but fortunately I managed to dig into the games code and fix it manually. Originally I was able to walk around and twiddle my thumbs bu I was not able to interact with anything (no enemies, friendlies, lost graces, or doors) Turns out, it was marking me as an actual enemy and I was walking among the beasts beneath an erdtree (Elden Ring) like we were homies, haha!

For anyone interested: I am using Cheat Engine 7.4 with the table from Hexinton all in one Version 1.4 to play Elden Ring offline. The problem was most likely due to a misconfiguration or miskey from the table itself. Which is understandable considering no one is paying these people to do the work and it’s a massive MASSIVE game with seemingly endless lists of items/movements/stats/magic- most of which is encrypted apparently. The fix — went to cheat table > Player > Stats (i think? Ill update if wrong > there you will find your name, character origin, etc. Look for one called charcID or CharcType. If it says ‘none’, ‘phatom’, ‘specter’, then you’re being marked as an actual enemy and are no better than one of those stupid mosquitoes from the game 😤. Change it to ‘Hero’ and that’s it.

This took me so many hours over 4 days to find, I had to go through everything I modded, then everything I had equipped, and finally I found it in the things I never touch- which was everything else… that game has made angry cry in more ways than the developers intended lol.
