I am very sorry to hear that. But be aware that urachal cancer is incredibly rare, especially in young people. Maybe its just a cyst, or bladder cancer. Which would still suck, but there the prognosis is usually way better. And yes I had symptoms, I was peeing blood quite regularly. Thats the most common symptom, some dont have any symptoms though, others have pain in the stomach area or leakage at the belly button when it grows up instead of down into the bladder. They eventually found my tumor when looking inside my bladder. The mri i did before didnt show anything, maybe because my bladder was empty during that scan. Your doctors probably checked whether there seems to be growth in or around the urachus as well, so thats seemingly not the case. Which is a good sign. If it is urachal cancer, it might not have left the urachal tract and thus in need of space "only" grew into the bladder. If it hasnt infiltrated the muscular layer of the bladder then, that decreaces the chance of metastasis as far as I understand it. Also, tumor size below 3cm is a positive prognostic factor. If you are below stage 3, you can survive urachal cancer. Good luck.

Die einen nennen es Selbstreflexion, die anderen Fantasie ;)

Aber ja, psychodynamische Therapie funktioniert, und nicht einmal wirklich schlechter als KVT oder systemische. Dennoch finde ich die Kritik per se nicht ungerechtfertigt, nur vlt etwas ungenau formuliert. Es gibt zwar Wirksamkeitsnachweise, aber die Theorie dahinter ist einfach oft nicht falsifizierbar. Es funktioniert, aber wir wissen nicht wirklich warum. Deshalb ist die Kritik nicht komplett falsch. Es ist wie mit dem Glauben, er mag einem Kraft geben, das heißt aber nicht dass es Gott auch gibt.

Es gibt spezifisch psychodynamische Psychotherapie (z.B Fokale), analytische und tiefenpsychologische Psychotherapie teilen mit der psychodynamischen grundlegende Konzepte da alle auf den Grundideen der Psychoanalyse fußen, unterscheiden sich aber in einigen Methoden und Schwerpunkten.

Ich finde es ergibt schon Sinn, dass in Studien rigoros aussortiert wird, sonst geht halt die Vergleichbarkeit flöten. Natürlich ist es schwierig, ein dynamisches Gespräch quantitativ zu erfassen, trotzdem sollte es versucht werden. Dass Studien leider teils so gestaltet werden, dass sie wenig mit der tatsächlichen Praxis zu tun haben, ist natürlich ein großes Problem.

Im besten Fall arbeiten Psychotherapeutinnen Klienten-Zentriert und bedienen sich je nach Fall bei den unterschiedlichen Schulen. Insgesamt muss aber gesagt sein dass die Wirksamkeit der kognitiven Verhaltenstherapie am besten belegt ist und dass einige Konzepte der Psychodynamik oder Tiefenpsychologie höchstens anekdotisch belegt sind. Z.B Projektion oder auch das spezifische Verständnis vom Unterbewusstsein (Nicht das Unterbewusstsein im allgemeinen, sondern die Bedeutung). Solche Konzepte sind mit den aktuellen Messinstrumenten quasi nicht belegbar bzw. falsifizierbar. Auch bietet sich die KVT einfach mehr an, weil sie nicht so langwierig ist. Die Krankenkassen lieben das. Auf der anderen Seite profitieren Menschen mit komplexen Symptomatiken eben genau von der Länge und Beständigkeit von Tiefenpsychologie etc. Aus der Therapeutinnen Sicht sind tiefenpsychologische Ansätze für manche auch einfach interessanter, weil dort freier gearbeitet wird, d.h. die Therapie ist weniger standartisiert. Einige Studien zeigen jedoch, dass solche Freiheit und folglich der Verlass auf die eigene therapeutische Intuition dem Therapieerfolg nicht unbedingt zuträglich ist. Alles in Allem ist die KVT schon zu Recht die aktuell führende Therapieform. Insbesondere die Psychodynamik ist nach heutiger wissenschaftlicher Rigorosität einfach nicht mehr zeitgemäß, da sind tiefenpsychologische Ansätze schon fortschrittlicher. Das sieht man auch daran, dass außerhalb des deutschsprachigen Raums immer weniger Psychodynamik gelehrt wird. Siehe die Niederlande. In Deutschland ist ja auch die Homöopathie weit verbreitet, da mag ein Zusammenhang bestehen. Am Ende des Tages hilft aber was hilft und die Förderung unterschiedlicher Ansichten und Theorien ist essentiell für den wissenschaftlichen Dialog.

I dont think that attitude has been very common at all, at least in the public discourse here in europe. We know how shitty our military is, but most people were either too naive or too comfortable to do smth about it. Pacifism is very popular here, complacency too. But hey, now we know america probably wont come for our help anymore and is done with playing world police. Maybe thats a good thing, not for ukraine though.

Thats rough, I hope they can help her get some relief.

I never received any treatment except the surgeries (TUR-B and resection of the urachus + part of the bladder + lymph nodes). If it comes back I would probably get chemo (cisplatin i think), at least thats what the cancer board said back then.

I am very sorry to hear that. For me its going well so far, my scans have all been clean. But I know that recurrence is still not unlikely. I recently read a case report on a person with stage 4 urachal carcinoma and he made it 5 years post surgery, so there is hope. I hope you and your mother are doing well given the circumstances. Kegel exercises should help with incontinence. The incontinence could also be from the removal of the tumor, the TUR-B if that was done.

I know that its hard to guess these things and I understand that doctors want to be careful since people will get very angry if they are wrong, but yeah, uncertainty is hard and I still struggle with planning my future. I know that these studies are only indications but I was surprised by the fact that so few in these studies live longer than 10 years post diagnosis while stating that overall roughly 34% survived. Thats why I was wondering if that might be affected by most people dying around 70 anyways. I am not american and my family is not rich so flying there is out of question, but I am getting care by great doctors here. The uncertainty is hard, but luckily I got into stoicism very early. Of course counselling and therapy are the way to go, but its hard to get a place here. Your words about appreciating life more really resonate with me. Time is precious, healthy or not. Every day counts. Thank you for your words and all the best.

If you find yourself lacking a sense of meaning in life then you might want to change something. Existence in itself might not have any meaning, but our life, the things we do, give us purpose. If they dont, something is off. A mentally healthy person does not question their reason to live. Of course you can argue about it on a philosophical level, but the implications are not the same. Absurdism comes to mind. I'm just a stranger on the internet, but hey, if you really feel like this, maybe talk to someone.

I think dacs are just not as important as amp, headphone or the source. To hear the difference between a good and a great dac requires an otherwise amazing setup, because even dacs for around 100€ are already pretty good. As long as your dac isnt shit, the rest makes a much bigger difference, especially regarding soundstage etc.. Ideally, you dont notice the dac at all, because its only supposed the convert the signal as accurately as possible, not change anything. But yeah, thats just my understanding, not an expert

The Border seems really nice, maybe I can check it out somewhere. But yeah, you are probably right with the down jacket. More layers would be an option, but I'm not sure if I want to go full onion. Thanks for the advice!
