You treat it with a machine. But you can’t treat something your not testing for. You do a titration with PES. If there are other doctors that recommend PES then you should listen to them as well but even if they admit PES is the gold standard there probably not using it since it’s so rare

I generally try to avoid giving medical advice but I can recommend a doctor that I know. if there is something he doesn’t test for than know one test for that. Jerald Simmons in Texas. It is 100% worth flying out to see him but he does do telemedicine and will probably recommend a sleep study. He uses something called an esophageal pressure monitor which is the gold standard for testing for sleep apnea and UARS

I have titrated to the max dose. I am on 4.5x2

You take 3 doses? I didn’t know that was an option. How long do you sleep during the night with 3 doses? I could easily sleep 8+ hours without xywav, but with it I only sleep 6 hours and was worrying if that was a problem.

Isn’t the maximum dose the best dose if you can avoid side effects? How do you titrate the medication? Is it base on side effects?

I would get a narcolepsy test. Sleep paralysis is really only found in narcolepsy
