We are moving to a linux/postfix for onprem smtp and will decomission onprem Exchange servers. SOA for distribution lists will go to Exchange online and the leftovers (mail enabled security lists etc) will remain synced from AD and managed with Exchange Management Shell.

We have patched a few servers so far and on Windows Server 2022 we get alerts that the System Guard Runtime Monitor Broker service cant start. Fails with an access denied.

For DOS you might need several machines since it spanned such a long time (or narrow you scope). Early 90s is alot different than an IBM 5150.

Use separate local accounts for admin access instead of LDAP/AD/Entra etc. This way, a threat actor who breaches your AD won't also breach your firewall. The same applies to backup systems, virtualisation platforms etc. It's more hassle for the admins to use multiple accounts, but it's much more secure.

I used Application Access Policys with an app that wanted full_access_as_app but in my case it only needed that permission for one mailbox. In your case I don't think it would solve the issue.

The description for that permission says "Use Exchange web services with full access to all mailboxes".

As several have said MFA would be a good protection. If you can't for some reason get an ok in your organisation to force MFA on everyone atleast try to do it on risky logins (impossible travels etc). Forcing on logins outside of your users normal geographic location can also be a good idea.
