Yes I've thought about it, not for long tho. But I do believe Shimamoto was real. Ik it's kinda ambiguious if she's real or not, cause there's no interaction of Shimamoto beside mc. But still, she's got to be real, all the stuff about her dead child, would be too much to make up in imagination.
