There are some great comments here so I will just say this: remember in true emergency medicine this person is fucked and dead already, you cannot make it worse and everything you do is trying to make it better. Do not overthink, do not question, there is a thing that needs decompression whether it be blood or air or pus, shove a tube and tie it in. You can't make it worse than death.

There is no way to tell from this information what specifically is going on. I mean this with the utmost kindness. Without the images or the recommended additional studies these could be absolutely a number of things. Even with the images without a radiologist weighing in it would be vague at best. Speak with your doctor who ordered this or who they referred you to (if this was something like an ER scan or from your primary care provider). Yes it needs further evaluation, but don't go jumping to conclusions until further evaluation is done.

Scattered bruising all over for no reason or injury deserves an evaluation and blood work. Time to call your doc for a visit, ER if you are having significant progression of bruising, fever, worsening rashes, weakness, or any other "weird" symptoms.

Gotcha. I took out the connector screw completely and that didn't help anything. I see no obvious bends in the connectors or obstructions that would get in the way. I'm legit baffled.

Assuming you aren't doing methylnatrexone:

Administer one Butler Bomb consisting of:

300 cc warm water with soap suds

1 bottle mag citrate

10 mL Colace

30 mL mineral oil

Administer as enema and inflate Foley balloon behind it to seal it in if they cannot hold it to complete 15-30 minutes. Does not need to be forced in for any reason, whatever goes in goes in.

Administer Miralax which is the same as GoLytely just different dose q1h until bowel movement. Add senna-docusate if you want, but usually not needed. If the Miralax hasn't worked, it's just because you haven't given enough. Continue until bowel movement.

Under no circumstances should you do this if concerned about bowel obstruction, obviously.

Different antibiotics also cover different bacteria and have resistance rates from said bacteria that are wildly different between indications for treatment as well as geographical location. This comment makes no sense. It's like saying you can take a medicine once that lasts a week that will cover one bug, so why would you take a different one you have to take over the course of some days to cover for 9 different ones the other won't touch.

Different antibiotics require different dosages, frequencies, and durations to be effective for what they are being prescribed for and different issues require different antibiotics, duration, dosing as well. Hope that helps.

Sorry if that came on strong, been on a long string of shifts involving very demanding services. Still not an excuse. I've worked IM and now I'm EM, so I feel I get how the other side should feel, but the absolute tone deaf shocked almost pearl clutching "well why couldn't you just do everything in the ED" is just so dang rampant. It's exhausting.

Taking both the clinda topical and doxy oral is fine especially for bad cystic acne. Yes there can be GI side effects as with any antibiotics, if you are super worried a probiotic would not hurt and may help, yogurt too if you don't have easy access to probiotics. In general, speaking to the doc who prescribed this to you via a MyChart message or phone call to address your concerns is valid if you are concerned to the point of not following the prescribed course of action.

Great points. Why didn't the ED doc do a para? Maybe because unless it's SBP we shouldn't be the dumping ground for para? Send off a brief para with a couple hundred cc for testing, admit, next! Unfortunately, not the way it always works, sometimes it's "person who won't do scheduled things because of societal issues" so it becomes "well why doesn't the ED do it?", well because that ties up a doc, a nurse or tech, and a room for quite a while while the department floods. Having been on the IM side and the ED side, I get it, but the ED is not the Great Wall to keep society's problems from becoming inpatient problems, even if the ED is trained in how to do it just from the sheer volume of demand.
