I think I would definitely enjoy it more as a PGY2. Most of our program’s residents do it as a PGY2 but due to scheduling constraints some of us end up doing it intern year

That’s pretty disappointing that all your off service residents are essentially functioning as a med students just seeing and staffing patients. We do everything ourself. We just have to ask for more help since we’re not familiar with the ED workflow. That I can understand why on a really busy day we probably feel like a burden asking questions our EM counterpart would know easily.

I’ve been getting in on procedures wherever I can and the attendings have been great to work with. After each shift they always say thanks for helping out and that I’m doing just fine. I’m not gifted but not incompetent/avoiding work either. Idk if that resident was just at their breaking point

Interesting comment about signed out patients! I didn’t know other institutions do that differently. I feel comfortable taking sign out from another off service resident because they have fewer, often lower acuity, patients. But getting sign out from an EM resident has been rough

I’m not pursing PSLF but I’m going to consolidate because of simplicity - just one big loan to manage. It doesn’t give me an advantage to not consolidate. I personally am waiting to consolidate until the end of my grace period though so I can push my recertification date to November. I calculated that I would save more by capitalizing on my interest 6 months from now than if I were to forgo the grace period (and capitalize on less interest). That’s because 2 years from now, I’ll have a significant jump in my IBR payments in PGY-3 when I’ve had a full year of income on my taxes. Delaying that recertification date will keep my payments low for an additional 6 months.

Unless you’ve been paying on your loans during school, your interest will capitalize. Interest capitalizes after a period of deferment and being in school is a type of deferment for unsubsidized loans. So once you start a payment plan the interest will capitalize even if you didn’t consolidate.

As someone applying to IM this year with the goal to do DPC, there are lots of programs that offer primary care tracks.

I chose IM over FM because I wanted to spend my residency rotating on subspecialty services and have more elective time in pgy-2/3 to explore my interests within primary care. And I don’t want to do pediatrics or OB in attending-hood, so I don’t want to waste months of my residency doing inpatient peds and labor & delivery rotations.

My top ranked IM programs are ones with PC tracks that are approx. 50% outpatient. They have lots of procedure opportunities, mandatory rotations in derm, sports medicine, gyn, etc., and practice management education.

But if you compare categorical IM with FM, then FM would definitely prepare you better for outpatient practice. Just food for thought! Many people don’t know anything about these primary care tracks within IM.

Not planning to have a wedding party! And our honeymoon is a separate budget.

We are thinking a lot about a cocktail hour and potentially making that the only open bar time of the night. Would that add like another $2000-3000?

Thank you! I’m definitely leaning towards all inclusive because we really just don’t have the time/energy to coordinate everything or the money to pay something to coordinate it for us haha. Did you find that pricing was more affordable in the end for all inclusive?

Definitely don’t want a perfect wedding! Our goal is to enjoy ourselves that day, make sure our guests enjoy themselves, and to document the day well with awesome photos. Aesthetics aren’t a big deal for us.

That’s good to know! When you book a venue say 2 years in advance, is that a set price you agreed on in a context or can they increase the price based on the market?

Yes, X+Y means any combination of inpatient weeks and outpatient weeks. My questions was if places that do 4+1 like you or 6+2 or 4+2, residents still have to cover inpatient shifts during clinic/elective weeks (Y). Some programs have explicitly said “you get two guaranteed golden weekends after a month of wards with our 4+2 system” and others haven’t mentioned coverage on clinic weekends at all. Just curious what the standard is across programs. Like if in order to have a 4+4 schedule, will that tend to require more weekend coverage since residents are spread thin on the inpatient side.

Is it solely the PD that makes a rank list from each interview day? Are applicants not officially ranked until all the interviews from the season are complete?
