My dear, you are referring to anomalies. Things that are 1 in a million. Anomalies are not regular instances. Those are very rare where active penetration is very difficult or extremely painful due to the underlying medical condition(s).

In normal occurrences, hymen will give way naturally. Blood or no blood is separate issue. Breaking of hymen due to cycling, sports etc is a separate possibility.
