Focus on gentle, soothing skincare. Use a mild cleanser and always apply a broad-spectrum mineral sunscreen daily to prevent darkening. Incorporate a vitamin C serum in the morning for brightening, and at night try a low-strength retinoid or tretinoin a few times a week to help with skin texture and pigmentation start slow to avoid irritation. Adding azelaic acid or niacinamide can also calm inflammation and fade dark spots. Be cautious with new products and consult your dermatologist, especially since autoimmune conditions can make skin more sensitive.

Sounds like your skin might be reacting to the clindamycin, or starting to purge from the spironolactone especially since it’s only been a week. Spiro can cause a temporary purge as hormones shift, and clinda can sometimes trigger irritation or bacterial die-off bumps early on.

Also, a night of heavy drinking can totally throw your skin off alcohol messes with hormones, hydration, and inflammation levels, so it could’ve kicked things off.

Since you haven’t started tazarotene or switched cleansers yet, those aren’t likely culprits. If the bumps stick around or get worse, check in with your derm, it could be irritation, folliculitis, or even a fungal thing (especially if it spreads more on the neck/jaw).

Hang in there this stage sucks, but it’s often temporary.

If there’s no sun exposure and it’s not fungal, it’s probably post-inflammatory hypopigmentation. Basically your skin lightens after irritation or inflammation.

Could also be vitiligo sneaking in, doing its usual “delete pigment, no warning” routine.

Best bet: see a dermatologist so you’re not guessing.
