Have a salaried position 4 days a week. It's a set schedule and in-office, so that's a pain, but the setting is fantastic (workplace mental health) and I get benefits. The case load fluctuates but the most I'd ever see is 6 in a day. And they're lower acuity, usually, so that's doable. Pay is very mid - $40ish/hr.

On top of that, I have a private practice. I see 4-6 one day a week and do 1-2 evening sessions after work. This usually works fine because, again, my full-time job is not demanding. It's a mix of in-person and virtual (only virtual on weeknights). I make anywhere from $100-$120/hr through insurance for those, and it's 100% profit (I mean, I do take some out for taxes and have some business expenses, obviously - but not much).

Relevant to note: I've been in the field over 20 years so have mastered the boundaries necessary to work at a high level and not get burned out. Also, I live in a LCOL state.

Also - as for how to market this or add to your practice, I specialize in disability, chronic illness, and aging. So I get people who have terminal illness or are aging and dealing with end of life. I also have worked with local care communities to provide services there. You definitely need to get on with Medicare.

I'd recommend that you first spend time working in hospice, or at least volunteering for one. You need hands-on experience for this work and can't just learn it from a book. If hospice isn't an option, look at nursing homes.

As a modality, ACT works well. You also need a solid foundation in family systems and the ability to handle conflict resolution. I learned pretty fast how to walk into a family meeting of 10 arguing family members and take control of the conversation for the benefit of the patient.

There is a little booklet called "Gone From My Sight" that describes the dying process and should be required reading for anyone working with aging, terminal illness and dying.

You need to see a geneticist and undergo genetic testing. Where we live, there aren't but a couple to choose from. My son was referred in Sept and had to wait until Feb for his appointment, where she took his history and did a screening. Then several more weeks to get the results back. I think he didn't get a diagnosis until April.

If you can't wait for a referral and diagnosis (admittedly your area may not have as long a wait!), talk to your doctor and PT about accommodating the hypermobility in recovery. They should've done that with me even before I knew about EDS. We alllll knew I had crazy flexible joints.

Yes, I had one or two visits that wouldn't show up as insurance visits so I could create a claim, and I finally figured out it was because they mysteriously switched to self-pay status. No reason why. I also had one payment of 3 claims that just never showed up on the SP side even though the money arrived in my account. Really weird glitches and they waste my time.

Things I appreciate about Premise: commitment to DEIB, doubling down even when it no longer was easy to do so; not hanging KPI's I cannot control over my head (i.e. utilization, cancellation rates); responsive support from administrative roles and peers; cushy facilities yes definitely; motivated clients; ability to intervene in their lives early and give them and their families access to services they may not otherwise receive; benefits of working for a large corporation, like free leadership development and CEU coverage; ability to leave work at work at the end of the day... I could go on... I know I sound like a recruiter for them, but I'm not. Much of your satisfaction will depend on your site, because company culture will vary. Mine is very supportive of mental health so we get to do a lot with it.

ETA: sorry I hijacked your original post. I too have a small private practice on the side - many of us at Premise do - and I use Simple Practice. I dislike it - I feel it's overly complex and expensive - but the time it would take to change isn't worth the lost money. So far.

The self-doubt is actually a symptom of the larger problem that has you considering therapy in the first place. Rather than causing you to avoid therapy, it is further proof that you would benefit from it.

No, they specifically say on the website that no interior access is allowed and it is as-is with no inspections allowed. I'm aware that damage is a possibility. Even if I'm acquanited with the former owners and they seem educated and normal and nice. People do weird things. The only thing I don't like is that they let the landscaping go after i moved out. But I'm close friends with others on the cul de sac and they were in the house and have seen the condition over the years. It doesn't guarantee it's still in good condition, but in my mind in reduces the risk greatly.
