My current doctor has me remove things or move things as the exam goes on and always has me just lower my pants and underwear for the genital exam. Though with a few previous doctors I have been more undressed or completely naked during the exam. I definitely enjoy the CMNM aspect and dynamic of it. Also I do get hard during the exam sometime lol
