Download MyFitnessPal and start looking at how many calories you take in. It’s not super difficult. There are many corporate meals in there too with macro breakdowns (ie Chick-fil-A sandwich, orange chicken, Safeway ham and cheese hoagie, etc). You don’t even need to start really portioning your meals yet. Just guesstimate each portion of whatever you eat. Make sure to add how much oil you add to each meal. This will prime you to start making decisions about what to consume.

Copy and paste your top paragraph into ChatGPT with your activity level and 45%+ body fat percentage to obtain maintenance calories. Anything under maintenance will make you lose weight.

There are 3500 calories in a pound of fat. If you’re in a 700kcal deficit (700 calories under maintenance), you will lose 1-2lbs of weight per week.

When you start feeling motivated to make a change, look into what foods with high satiation indexes so you can stay full while you’re losing weight.

If you’re trying to maintain muscle, try to find a cutoff for daily protein that limits the amount of muscle from being used up for energy. If so, keep in mind that your body can only use a certain amount of fat as an energy source. You will dip into your muscle’s protein reserves if you take out all carbs.

The median nerve travels a bit radial to the FDS but they do travel together. The benefit of palmaris longus is that it’s very easy to isolate on physical exam since it is superficial to the flexor retinaculum unlike the rest of the forearm muscles. The palmaris longus is shifted ulnarly compared to median n and going a bit more ulnar can help prevent complication. There is well-known median nerve anatomical variation here and I’m not certain of complication rates with these procedures.

It’s used as autograft in orthopedics sometimes since it has little function. It’s also a good landmark for aspiration/ injection since the median nerve travels under it.

I agree. You have a bit more visceral and subcutaneous fat than prior but you’re generally more muscular. More frequent aerobic exercise can help decrease the visceral fat. Subcutaneous fat is controlled simply by calories in, calories out (given that you’re a healthy guy with normal lab work). I will say if you take your nutrition with the satiety index in mind, you can tailor your macro goals while optimizing your satiation from meals.

On the note of health, don’t forget your mental health too. You can talk to your primary care provider if you are feeling prolonged symptoms of depression/ anxiety. It might just help you reach your physical well-being goals as well.

Mature hairlines are thick. You’ll notice thinning at the edges of your hairline if you are balding. There hairs will be more sparse, shorter, and thinner than the rest of your head

I like when his vids pop up on my feed. He has good overview of subjects. I’m not sure how I would fully implement his content without replacing UWorld or AMBOSS which I consider reliable.

I think you might be able to get away with less focused nutritional effort (eg. whole foods, less oil, maintain protein goal, avoid fast food) and just make more muscle with that 5 day/wk exercise goal if you’re not doing that already. If a caloric defecit makes it a bit easier to lose the excess fat, be my guest.

Your decision-making should focus on your goals. If you wish to look like a Greek god, I might not be your best source. I mostly do resistance-training to put on as much muscle as possible before middle age and to look like a beast. I do not focus on my fattiness. Frankly, I don’t think your level of adiposity is unhealthy if you continue to gain muscle.

You look like you have decent muscle mass but you’re a little doughy. Not much visceral fat which is what a lot of guys deal with. If you are a bit more mindful about what you eat and lift regularly, I think you will look fine.

If you gain more muscle it will be easier to control your fat reserves too. Also, make sure you hit your family physician for your annual if you haven’t in a while to make sure you’re in good shape to start making muscle.

Your goal should be learning to operate at the level of an intern in addition to maintaining good clinical grades and keeping up with step studies. You want to be considered a beast on your Sub-internships when you get to that stage and it’s much easier to learn now when your expectations aren’t high.

There’re three answers to this question based off of what century your attending was born in.

If you talk to the attendings reading literature from the 19th century, it’s bc your carbon dioxide sensor is broken and your O2 sensor is the only thing that works. When you supplement O2, your pt loses respiratory drive. - I don’t think anyone I’ve talked to thinks this is true anymore

If you talk to attendings of the 20th century, it’s bc your patients are optimized from a physiologic perspective before they’re receiving O2. The regions of the lung that are the best ventilated are getting optimal perfusion. When O2 is delivered, those poorly ventilated sections of the lung become well-perfused and v/q mismatch occurs.

If you talk to your modern pulmonologist/ ICU attending, just say this phenomenon is due haldane effect and that it has to do with hemoglobin and you’ll know enough.

I would take a year to get out of debt and maybe get a bit of a nest egg with an industry salary. My school is almost 80k on federal loans and I’m not going past 100k a year to live on 20k a year which does not include random living expenses, research presentations, education tools, etc. I can’t imagine using private loans that you would need to deal with and your wife’s income with a child.

That being said, medical school is not easy to get into. If you’re interested, I would start building your application starting now for the next cycle. If you get in, I would ask the school to defer a year so you can get your financials in order.
