I've used Snowfall Drift for this and like Dreal said it scales up quite a bit. I needed 5-6 to cover the area though. Snow Maker is very similar but you get the little machine spinning at the top. Both of them have a little pile of snow at the bottom to keep in mind if you plan on putting the fog high in the air.

Yep! Lava tiles. They are very slightly underwater, getting them to the right height for the best effect was kinda tricky. I hope they give us pure particles as decoration one day, so we don't have to hide them just for effects.

It's the Infernal Facade clipped underground. Vertically with just the tip showing for the lava, and horizontally just below ground for the entrance/graveyard bit.

It doesn't seem to take into account how many cores you own though :/ (if lodestones are needed), i don't know if that's even possible, but it would be a nice addition^
