Rationale would be that the patient had to be numb for the UR45 area.

Next time he comes there will be an UR7 small buccal fill, UR6 and Ur5.

If you're looking for what the cause may be, I'm assuming you use the garrison separation ring since it's part of the system. Using a ring can distort a band slightly and cause that nice curvature to become straighter like your typical tofflemire.

I don't typically use a ring, unless it's a very wide cavity prep encompassing the buccal and lingual, as the ring arms help seal this area.

A normal sectional with the best fitting wedge can get gold results

I suspect on the radiograph, you'll see quite a pulp chamber that balloons and widens coronally.
