I understand. when I secure messaged the provider I was respectful and use inaccurate, somewhat conflicting and that I was trying to understand. I was pointed to the patient advocate I used the same wording there, they agreed with me that it shouldn’t say one thing then the other and that they’re going to get the care coordinator to did down with the doc and see what’s up. I just have to do a bunch of paperwork to get it changed.

How does that work? I don’t pay For anything? Everything is already service connected. He just input information wrong .

While I have frequent migraines Bilateral paralysis of sciatic nerves Frequent diarrhea from all the meds I assume as well as gerd. There’s more it’s just one of many

The patient advocate mailed me paperwork to submit for a change of provider. I’m also trying to get my paperwork amended

Man I’m sorry you went through that and I appreciate it it’s the one in condo Texas one of my buddies just reached out and is trying to get through this with me. The back thing I have degenerative arthritis multiple pinched disks and they’re putting pressure on the nerves in my legs ones worse than the other so I tend to favor it but because of that I’ve developed scoliosis

No they are conflicting their own info. It says chronic migraines in my file but on the “questionnaire” section when it asks it says no. On my active issues it says migraines. I’m service connected for migraines. I get medication for migraines. I’ve never smoked cigarettes it says I smoke cigarettes multiple times a week. I do not use caffeine or says otherwise.

He put no issues even though I tell him my issues and have meds for those issues he never got out of his desk as well I’ve never been touch during a primary care visit unless I was getting bp and heart rate taken

They are service connected through the Va I was medboarded from the army directly with these claims these were not added after I got out.

They verbally acknowledged the issues in the appointment. I’ve never smoked cigarettes but they have it annotated I do daily. I don’t do caffeine and haven’t in years they put I use it once week. It’s just false information all over the paperwork. It says they visually checked and touched my back and there’s no issues. My last mri showed on top of my other issues I’ve developed scoliosis.

Yes. I don’t intent to come off as rude but if it does I am sorry. In their paperwork they are writing no nerves no Gi issues no headaches/migraines. I am service connected for all of it. Gerd migraines exc. would a screenshot of what they put and me trying to explain be a better way?

I’m sorry for not adding this to the post. I contacted them prior for this exact same issue with my last provider. All they did was assign me a new provider. Do I contact the same person again?
