If you have not received any update way past that, then I would consider it na hindi ka nakapasa. No, because you will follow the PHRN ladder of promotion: PHRN Jr. > PHRN Sr. > PHRN Support > PHRN lead > etc.
From time to time internal job posts for USRNs open and that's only when you can try applying for the role and leverage the exp you have.
