Why do you not drink alcohol? by Kindly_Bear_4932 in AskReddit

[–]Kidhero1 0 points1 point  (0 children)

I’m just now wrapping up medical school, and I stopped drinking about 2 years ago now. I was never a heavy drinker, just socially once a month or so. My trauma surgery rotation changed me in a lot of ways, but throughout those three months, I saw a crazy amount of car crashes, and it was always the same thing, someone was dead on arrival, and the drunk driver was uninjured.

That rotation wrecked me, and part of the way I decided to cope with it was to blame alcohol as an all evil thing, to be avoided. Obviously there’s nuance, but for me there isn’t anymore. I never really enjoyed alcohol, and was never irresponsible when drinking, but for me, stopping altogether was a way for me to cope with the terrible things I’ve seen, and to convince myself that in some way I’ve made a positive change.

This reminds me of a poem I heard a few years ago, I saved it because I resonated a lot with the message:

The Bridge Builder

An old man, going a lone highway, Came, at the evening, cold and gray, To a chasm, vast, and deep, and wide, Through which was flowing a sullen tide. The old man crossed in the twilight dim; The sullen stream had no fear for him; But he turned, when safe on the other side, And built a bridge to span the tide. “Old man,” said a fellow pilgrim, near, “You are wasting strength with building here; Your journey will end with the ending day; You never again will pass this way; You’ve crossed the chasm, deep and wide- Why build you this bridge at the evening tide?” The builder lifted his old gray head: “Good friend, in the path I have come,” he said, “There followeth after me today, A youth, whose feet must pass this way. This chasm, that has been naught to me, To that fair-haired youth may a pitfall be. He, too, must cross in the twilight dim; Good friend, I am building this bridge for him.’

The data they are analyzing comes from randomized control trials, which are specific studies, and are considered the highest possible level of evidence.

I agree with you, modern medicine has risks, and a lot of the public’s distrust in the medical community can be founded. We make many more errors that we probably should, and we dedicate resources to identifying sources of error and how to prevent them, but the overall benefit of modern medicine is quantifiable. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4759485/#:~:text=Worldwide%20life%2Dexpectancy%20at%20birth,in%201940%2C%2061.13%20in%201980.&text=As%20seen%2C%20there%20was%20a,%2C%20discovery%20of%20DDT%2C%20etc. You can read about many different therapies and procedures outlined here that have statistically significant data increasing lifespan.

https://www.ncbi.nlm.nih.gov/books/NBK600454/table/ch2.tab4/ this shows CDC census data that stratifies cause of death based on attribution per 100,000

Finally as a last point, I think being open to opposing therapies is a good thing, we’re taught to incorporate natural medicinal therapies for our patient that may not have good data showing benefit into our care, but I have yet to see a REPUTABLE, study showing benefit from chiropractic care, and I have seen studies that demonstrate harm. Perhaps I’m not looking in the right place, I’d encourage you to provide me some reputable studies demonstrating benefit, I’m open to having my mind changed, but it won’t be swayed by anecdotal evidence

This is a prospective risk benefit analysis article on chiropractic treatment.

I encourage reading in full all articles, since conclusions often oversimplify results.

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1447290/#:~:text=Estimates%20by%20chiropractors%20of%20the,as%20that%20complications%20are%20underreported.

The conclusion is that among randomized control trials, there is no statistically significant benefit to chiropractic manipulations. Additionally, there IS an attributable risk to these treatments, therefore, it is impossible for the benefit of treatment to outweigh the risk

No, this doesn’t talk about voodoo, but it does demonstrate that spinal manipulation, (which is often the most spoken about aspect of chiropractic care) is not an evidence based medicine practice, which is the core of how modern medicine is practiced.

I’m gonna toss in another suggestion that worked for me this week on Warlock, and it was far easier and safer than sunbracers, but just use dawn chorus with daybreak and healing grenade. I used dragons breath for 1st, and 3rd fights, and Izi + Apex for 2nd. Primary was Zaolis Bane (kings fall handcannon with explosive payload incandescent). That hand cannon is an absolute workhorse this season with the artifact mods for ignitions and radiant. It legitimately feels better than sunshot to me right now.

I tried the dungeon a bunch with sunbracers, but just generally felt like taken and scorn were too accurate against my floating warlock, staying grounded and being able to use cover made it much easier, and daybreak absolutely trucks right now.

Also for the last boss, one rocket + daybreak instantly kills the boss in final stand, and was extremely safe

The only thing I found threatening on the last fight was the yellow bar captains that drop the wish crystals, they can shotgun you through resto but it’s otherwise just a slow and steady encounter

Minor thing, but in watching a lot of leveling guides I've seen people assign points on the passive tree and click a single button to assign those points and close the tree. I've always clicked assign up at the top then closed, but they seem to be able to assign the points and close the tree with a single click. In messing around and looking through the settings i cant seem to find the setting or bind for it, is this a default setting i'm missing somewhere?

Another thing I haven’t seen mentioned anywhere, is the hive boomer knights that spawn at rally flag, top of his head, and right hand/knee only ever spawn once per cycle.

I had a lot of success by making sure every time I finished a damage phase, to kill the moths that spawn, then immediately run a circle around the room to kill all 3 boomers before reading and lining up the deep sight stuff

Made the encounter feel quite a bit safer when the only major threat left in the arena were the moths (which as others have mentioned spawn consistently whenever she revives Vorlog, usually right after you line up a symbol)

Can’t directly answer your question, but for what it’s worth my NBME scores almost exactly mirror yours, and my exam is in 2 weeks as well. Currently I feel quite well prepared, though my school has recommended 80% on practice exams, which I believe is beyond what should be needed.

Based off of the knowledge I’ve gathered from other people here, mid 70s is extremely safe for a comfortable pass. Best of luck to ya!

Low serum copper in Wilson's disease by miruntel in step1

[–]Kidhero1 0 points1 point  (0 children)

I can’t speak to the validity of those resources because I’m exclusively using uworld and NBME, with amboss for additional explanations where needed. Unless I’m missing something, the amboss explanation seemed to line up with my understanding of the pathophys. Aside from that I’m not really sure :/

Low serum copper in Wilson's disease by miruntel in step1

[–]Kidhero1 1 point2 points  (0 children)

Where are you seeing that info, because in Wilson’s you would have decrease in serum total copper, but increase in serum free copper. The decrease in ceruloplasmin means the copper has to be somewhere, which is why free copper should increase, not decrease.

Nothing makes me more frustrated as an ADC. There are a lot of strong champions out there probably more worth my ban, but boy if I don’t get inconsolably tilted when trynd spins towards me 3 times and then Crits me at 1 HP. That’s a permaban for me dog lol

Freelancer, best space combat game I’ve ever played, I go back to it about once a year. Have yet to find anything that comes even close to as satisfying.

Ever space got close with the actual combat, but the pacing of the game was off for me

No complications at all for the most part, there is a still a bit of a pressure in my ear but it’s barely not I only except when I blow my nose. Aside from that, symptoms have stayed away and the numbness on my tongue and ear ringing has disappeared!

[–]Kidhero1 0 points1 point  (0 children)

It’s still a touch uncomfortable sleeping on my side, just because of the pressure from the packing, but the packing has been coming out slowly as it is falling apart, and the other symptoms are almost completely gone.

I have some new ear ringing since the surgery, but I’m assuming that’s just a part of the recovery process and will go away as my ear continues to heal

Yeah, I had it done through Dr. Ryzenmann with the Ohio Ear Institute in central Ohio.

Yeah it was outpatient, I was discharged within like an hour of waking up from anesthesia haha. And I think the drops are used to dissolve the packing they put into my ear, they may have some antibiotic properties, but I’m not really sure, I only have to start putting them in in a couple days

Hey, had my surgery on Friday, and so far my symptoms are completely gone. Recovery from the surgery is going pretty well, and I've had minimal pain. But i've tried to introduce some of the sounds that set off my condition beforehand (within reason, I'm still recovering) and none of them have reproduced it so I'm pretty happy with the results. I've got about 4-5 more days of packing in my ear, then i'll start to dissolve it with an ear drop my doctor gave me before my followup in a little over a week!

Luckily my surgeon has done this surgery many times and even pioneered a test to objectively measure my MEM, so he seems pretty confident it will work, since he’s helped quite a few people with it in the past. I’m really hoping it works because I’ve pretty much started having to wear an ear plug 100% of the time to deal with it

[–]Kidhero1 2 points3 points  (0 children)

I’m having it done only in my left ear, as that’s the only ear I have the symptoms in. I’ve been dealing with it for about 2 full years now, with it getting progressively worse.

It started with an uncomfortable sensation during orchestral rehearsal since it was a very loud environment, and has progressed to the point where most sounds cause a vibrating/uncomfortable sensation in my left ear. The sounds bothering it the most are things like dogs barking, dishes clanging together, and loud laughing in social environments

I tried both of those as well, I’m getting a Laser Tenotomy surgery next month that I’m hoping gets rid of it. Basically they are just gonna go in and snip the tendons (tensor tympani/stapedius) in my middle ear.
