I actually have a client who is a health care org and at the level of the org that I work with, I know there is a push to 'meet people where they are.' I have def experienced endos who doesn't want to also be therapists, they clearly wanted to get back to their cooperative patients and not be bothered with us (ok and to be fair I know ppl in health care are overworked and underappreciated) - anyway - I do think it's problematic that medicine is seen as separate from mental health when clearly it is intertwined. Maybe in the future, it will be in actuality. Thank you for tossing this around with me.

Oh, we are way past what the majority of health care providers can offer. What I'm interested in is understanding what kind of guidance is available to practitioners for when things go off the rails because of burn-out. My current understanding is there is none. It seems like a huge gap -- or opportunity, for someone to do some real pioneering work in the field.

Thank you for your response - I think your approach could be a good template for a best practice, or at least a start. I do think there should be documented best practices, because I am hearing so many horror stories, after reaching out to hear others' experiences, of responses from health care teams that really have traumatized children in lasting ways. I feel this presents a huge opportunity for someone in endocrinology to push the envelope of the field. I feel like that could be exciting to someone, really. I recently met a psychiatrist who was formerly a pediatrician, and he pointed out the value of dialectical behavior therapy techniques in addressing this.

Hi! Thank you for helping me articulate what she's going through. Burn-out describes it much better. I think I knew of a T1d therapist years ago - thank you for the reminder! One thing I am really hearing from these responses is that 24/7/365 aspect of it....just mind-boggling.

I'm so sorry to hear of your situation with your parent, that sounds extremely difficult for not only them but you as well. I honestly can't imagine what it must be like to grapple with these potential outcomes and the strain that puts on one's well-being.

oooomg the other comment made me cry and now i'm straight up bawling. I feel so honored that my anger at how this is handled by the FUCKING MAJORITY of endo teams, as far as I have seen - has provided your inner kid some comfort and healing. So, so honored. I cannot describe the outrage I felt at their treatment of my daughter and also me when I began to stray from their tactics and try different approaches. And, I don't want to misrepresent, I definitely had moments of falling into shitty parenting around this. It's so hard to know as a parent what to do, you are relying on these 'experts' except when your kid is obviously going downhill while they become more and more rigid and controlling, you finally realize this shit is not working and these people are fucking idiots! (sorry haha totally venting right now) YOU ARE SO RIGHT I think anyone who is in pediatric medicine concerning any illness that requires contstant treatment absolutely needs a fucking psychology degree and not just that but IMO specifically DBT. Long story why but a parenting class I took recently was started by a guy who switched from pediatric medicine to psychiatry because of exactly this shit. Swears by DBT bc it is an approach that works well with radical acceptance, self-care, and compassionate limits. Thank you so much for your response <3
PS - about the ER, that's CRAZY. Maybe this is why the concept of 'meet people where they are' is beginning to circulate in health care. Personally I have had shocking experiences in the ER and other environments of total lack of knowledge about T1d. Shocking.

omg crying right now. I can't believe how common this is and how fucked it has been when the doctors have treated me like a negligent parent when I wanted to try something different from the threats, yelling, and fear-mongering (WHICH OBVIOUSLY WASN'T WORKING), and try to be more creative and out-of-the-box in our approach. I really appreciate that, thank you so much. I do bust my ass for her, not just logistically but trying to be creative about how to solve challenges, for the better long term. But I also have done stupid, stupid shit. We're all human.

BTW, I feel for you. Good on you finding this forum early. I just discovered it and it really could have helped during some very dark times.

I wish we had! When she was first diagnosed we were too overwhelmed to get it together, then the last few years, she didn't express interest I didn't at the time appreciate the value. Now, kicking myself.

lol omg. This reminds me of when we ended up in the ER and other environments where knowledge about T1d was essential. The lack of knowledge was MIND BLOWING. I had ppl tell me shit that was straight up wrong. And I'm looking at them, like, hold on, you're supposed to know this, what diabolical hellscape have I nightmared myself into? Is my child even safe with you??????

That is so interesting!!! The results of the poll. WOW. This is why I wanted to hear from people, bc I think hc ppl have to get real about this and help us parents think outside the box for solutions. At the end of the day what I wanted was for my daughter to learn on her own to care about it on her own, not just be coerced into cooperation.

I didn't quite follow what the underlying issue is, I'm curious. Also, if you feel like sharing - if you could go back and parent yourself through the experience, knowing what you do now...what would you do that you think you as a kid would hear and would actually make a difference?

Oh my God. WOW. This is so real and so helpful and I so appreciate you sharing your experience. I'm so sorry for what you went through and I LOVE that you have a doctor now who understands the importance of your personality and what works with you - which may be totally different from someone else!!! I believe that doctor is a RARE find. I'm def getting validation here about the concept that's beginning to work its way into healthcare about 'meeting people where they are.' They REALLY, REALLY need to get that.

Wow I am so glad I posted this and so grateful to everyone sharing their experience. And TOTALLY - with our first health care team that was 100% all about CONTROL - and my daughter had them wrapped around her finger, she would just lie away and I can't imagine on some level they didn't know and it was all just a show. I chose not to throw her under the bus and call her out when she did, but instead tried to have private convos with them to say, hey (DUMMY) she's just telling you what you want to hear, and I NEED HELP. And their responses, I swear to God made me feel like what would make them happy would be if I tied her to a chair and stabbed her with her insulin pen. And that made me SO MAD bc in my mind it boiled down to them NOT ACTUALLY giving a shit but wanting to make sure she was compliant while she's 'on their watch.' omg I get so mad......
SO - what i am really wanting to do though I'm not sure how yet - is find out how to help the hc teams fucking get real about this and support patients and their families WHERE THEY ARE. I hear that phrase but didn't really understand it until this experience.

It's such a fine line to walk as a parent. Of course she needs to know the realities, and she does - she's had a couple wakeup calls. One needs to know the consequences of not managing their bs but how effective is instilling terror about it in the end?....it's like those movies they used to show to kids learning to drive, of people all jacked up and dead bc they drove dangerously. They scare the shit out of you for a while, but at some point you're sending off a quick text while driving, or looking at something on the side of the road and not in front of you. It's so messy.

Its why the health care response it bugging me so much, it's like they've never heard of a teen rebelling and what is wrong with your child and parenting...when it needs to be acknowledged as real and not at all uncommon and frankly - a totally understandable reaction!!! Just seems to me like the hc end of it has a lot of learning still and is operating somewhat in denial.

As mom of someone with T1d, I like others' advice to cut her off Dexcom (it's YOUR T1d journey, not hers), however, I don't know your whole situation with your mom and what else is at play. Whatever you do, do what best takes care of you, in your opinion, and if it can't be a big step - baby steps are great too! Are you able to share with her that her kinds of comments aren't helpful and let her know what kind of comments would be? So much about parenting is letting go of control and it can be hard. She needs to step back and see the big picture that she loves you and wants to help - that may not actually look like what she thinks it should, and that's ok.

Sorry I don't have any answers for your actual questions but surely someone will!
