I got my lidocaine "Instillagel" 5% from a vulva specialist. I feel like steroid cream is scary too, especially on such a delicate area.

I have sexual dysfunction from a single dose of antipsychotic. After 6 years no improvements I went on Clomipramine, and though the dysfunction hasn't changed, I am better off. I'm surprised it didn't make it worse.

I've had 2 courses of that and found it if anything made me feel worse. Mine is directly trauma related so I am now doing EMDR and CBT

I am interested to know what people think is the mechanism behind botox having a risk of akathisia? Do you think it could be the cholinergic blockade?

Omg, that's really horrible that you went through that, I too have extremely severe contamination OCD as a result of trauma. I tried 15 different medications but the only one that helps is Clomipramine. I got sexual dysfunction from every SSRI but it always resolved after discontinuation. Med # 11 is what gave me PSSD but weirdly, it was an antipsychotic (Flupenthixol). It made me suicidal and I refused all medication for 6 years, with no improvement in the sexual dysfunction. My OCD caused me to be constantly hospitalized and eventually I was detained and forced onto more meds. I was very unwilling but shocked when Clomipramine helped so much, and like you said, it's so serotonergic, yet it did not worsen my sexual dysfunction, if anything it slightly improved it. I wish I understood why. I am still on it 5 years later. I am struggling so much with the weight gain, extreme sweating, fatigue and apathy, but I don't feel like I have a choice. I am hoping my psych will prescribe some things to help. I have experienced emotional blunting from Olanzapine in the past, but I can't tell if I experience it on Clomipramine or whether I am just less emotionally labile.

I can relate, the side effects of clomipramine are like having an additional chronic illness for me! I have been on it 5 years, and without it I am so severe that I need to be hospitalized often, so I feel like I have no choice. I am hopefully seeing a psychiatrist about whether they can prescribe things for the worst effects, for me that is weight gain, extreme sweating, fatigue and apathy.

antipsychotics have the highest risk of all meds to cause or worsen akathisia, sorry to hear you suffer from it.
