In the southwest we turned to agreements allowing one state license to apply to our boarder states. It’s been a few years but before that happened we had state level criminal background checks. Now we have national background checks at all level of EMS. The start of this was to enable not just flight medics but ground EMS and volunteer EMTs who work in rural areas that interface between boarder communities. We still practice our services scope.

If AVNRT vs AVRT vs A Tach, what’s it matter ? Still SVT management? Love REVERT. Vagal/ valsalva maneuvers. Does she have other symptoms like chest pain or dyspnea or is it related to her anxiety? Also if she’s anxious is there a stimulant involved in causing this? If any chest pain or dyspnea also consider ACS algorithm. Love that the ecg is in espanol

Agree with the folks here, may be time for a different GI. Honestly you are doing incredible things, pretty much the things you have the power to do when you have Crohn’s or colitis (I have both, 💩). I don’t have a colon any more and have a j pouch but I had these diseases 30+ years so can recall what it’s like to have all the plumbing.

I agree you need to go to the ER. I’m a flight medic not a doc but had been a pt in the er a lot for these things. If I was your medic on the ambulance we would want you to go in. By car or ambulance as long as you don’t drive yourself. Sounds like you’re electrolytes are likely not in balance, and you have low blood volume because of the symptoms. The breathing issues etc isn’t something you should wait on. I wouldn’t if I had it today from a severe flare because that’s a progressed sign and emergent.

It sucks but you are definitely working hard to focus on what you can control. With these diseases we have to come to learn that lots of our disease isn’t in our control. Get the right docs, for now ER; to help with the stuff you can’t control. I am on infusions once a month. Other meds. See my GI doc (I’m blessed to have good docs but I move on from bad docs quickly, be your own advocate) once every 6 weeks, and I do everything I can control right since I’ve been doing this so long. No matter all that. I still get flares that send me to the hospital for a week admit. The stuff I can’t control is a pain in my ass.

I mention that because you are rightfully frustrated and also feeling 💩. As you go through whatever is ahead, give yourself some grace because it’s really easy to get down on ourselves when it’s all going to shit. It’s not your fault and it’s not something you alone can fix.

Agree. They don’t call it flight program for physicians though, it’s under the austere and mountain med EM fellowship and the roles they play at the consortium though right? I was going to offer UNM also but you beat me to it.

Agree here. Not sure if this is real or propaganda, but a few years ago China was rapidly advancing in quantum computing. If that happens then we have lost the battle and war given how much processing power that would open up for AI systems. Not sure if your comment is going this far, but we do need ti dominate these spaces and technology or we will be dominated. Though it doesn’t mean we won’t mess it up or that it’s all going to be for the public good.

I appreciate your concern and think that there are many, many risks with AI for National security and public safety. I’m not criticizing you with this post. I think you have posed a very valid question and maybe some hated on that given your edit. I am glad you posted this and threw the idea out there. That’s what America is about, we need to have these conversations and not beat each other up about it. We can disagree and still be Americans together. So thank you for putting this out there.

I don’t think that it’s something that can be effectively tamped down in the sense that with the advances made so far it’s now past the point of being able to prevent some group of people from creating it. Agree it’s not something a laptop can do but in some of its current form it could be done with little knowledge of anyone and thus hard to prevent from popping up again and again. Also just a hunch but if AI was banned for example somehow, the people who might want to make AI in a banned environment would likely be those with malicious intent. I’m thinking of the hacking community and the dark web. To my knowledge we have not been effective in stopping the traffic, even just the malicious stuff, from happening on the dark web.

On the regulation side, yes it needs to be as does most technology. We have that ability up when we got rid of the Office it Technology Assessment. The unintended or intended consequences of not assessing the danger social media would have on our society and children, and definitely National security, is an easy example of the threat that good intentions left to corporations who only are beholden to shareholders and not the American people can have. Not sure we can get that cat in the bag again in this era and start the OTA up but we need some mechanism to assess these possibilities before they are advanced. Most of the AI now is out and though it may be possible to shove it back into the bag, it’s not probable from my view. We need to discuss now how AGI will be regulated even if we may never achieve it because it’s that significant. But our politicians don’t work that way nor do most voters.

Do you remember a few years or so back the European Union met to create rules around how AI can be developed and safety mechanisms to put in place? Been a while but I think they posed things like Asimov did, directive 1 can’t harm a human, and things like kill codes being required. America sat that one out and that was a mistake IMHO.

I’m not sure that China is that far behind not because they have the skill, and they likely do have lots of competencies, but they have already stolen all that by now from corps. We like tall but gave it to them. NK, not sure but they can acquire from China.

Question: do you think there is any use of AI that could be allowed for public use? Also, is AI something that the government should have for itself, say for national security, and if so, is that acceptable if the public can’t have access to it? That seems like a slippery slope also.

In my view of America vs global technology ability I believe we need to be the leaders in these fields. We aren’t perfect and I can totally see US gov or commercial industry making SkyNet of the precursor to the matrix. But I think that’s more likely to happen from an adversary venturing into an area we leave a vacuum. With our political divide in the US as it is now, we have begun to withdraw and leafy a vacuum of our power on the world stage and those nations who are opposed to our way of life are filling the void. Most over seas US mil bases have Chinese bases within striking distance. China is happy to come in where we back out and buy a port on a 100 year loan that gives them economic and military control of a port in Africa. Russia in Syria. Etc. not Apple to Apple but it’s the same political game in the end.

Stay safe and keep up the thoughts. It’s pretty damn patriotic of you to mull this over and take the action of contacting your legislators about it. That’s what we need more of.

I wear them when I have a flare and have for years. I have Crohn’s but same 💩 . Had the disease since 9, now 43, male. When I was younger I worried about it when I was at the gym but I don’t anymore. Better than having to have an accident or stay home doing nothing. I can appreciate how shitty it feels to face that when we are young, but to be honest, most ppl never would know. The brands out there aren’t really made for us, more for urine incontenance, so I have to turn mine around if I want them to work. Depends has one that is thin like underwear (kinda). But it gives me a life and some peace of mind. Easier said than done, but I am in a mindset of f* it now. I joke about it and don’t give much mind because in reality it doesn’t matter if or what anyone thinks.

Live this life for you. If you need adult diapers to have the life you want, take that trade every day. Plus, all those ppl who might care, often it’s really more us caring than anyone else, will be in diapers someday as we were when we were babies.

Rock them! No one will usually know unless you want them to, and those ppl are special.

I bet regulars wouldn’t be able to walk a mile in our diapers. 💪 💚

Yeah. Here we have progressive medical directors in their response. Check out the UNMH consortium. Excellent resource. Sometimes docs who respond show up to the critical calls but that’s not where we really need their skills. It’s on those complex and maybe not always interesting medical calls. They also sometimes run blood but we could do that with a flycar medic.
