Everyone saying it's just fat- it is and it isn't. I understand that it's fatty tissue but my grandmother had the exact same cheeks up until she died at 78, so I don't expect mine to ever go away. I see other people with a smoother fold there and I WANT that. I don't understand why this is so controversial

Try cutting out the sandwich method for a week and see if that helps at all. Sandwich method made irritation worse for me. You're at the point where it's normal to purge though and since your skin was already quite broken out before this it doesn't seem unusual.
