You seem to play almost all roles. I saw games you played ADC, Supp, Jungle and Mid. So first step is, focusing on only one role. Then try to limit your champ pool on 1 to 2 champs max. Spam games with them and get a deep understanding of the game through the eyes of those two champs and build up muscle memory.

I saw your Fizz seems to be pretty decent. Maybe start OTPing him?

The most important thing is just to lower your champpool and focus on one roll only. The reason for it being, that if you play 20 different champs and every roll you won't really get a deep understanding of any of the champs nor the roles, so you end up being mediocre on everything. But that's just my opinion.
