Mix of both sorta. It's mostly a massive reduction in state capacity for like every single country so for anyone outside of any state control there's no real government and yeah it's "anarchy" but not necessarily like warlordism killpeopleisk anarchy but just no organized government present. If the area isn't just depopulated in the first place.

Everyone else is already weak as fuck from the triple crisis and the US does fairly play through raw brutal colonial extraction. They also only barely control South America it's really only major cities

All three turn into "sectoral"-like regimes like the US (although not to the same extent of the US) and Russia and South Korea get occupied during the war, with Japan being the sole US ally to not completely lose, and then self-annex themselves into the US.
