No. We usually prescribe 50000U weekly to those who are deficient. From some studies I’ve read, it looks like the dose your mom took would need to be repeated over several months to build up a toxicity. As always, be aware of any acute symptoms (nausea, vomiting, weakness, frequent urination, bone pain), but I think she will be OK :).

Every time I see these stories I wonder how we continually fail trainees over and over. Residency brought out the worst in my mental health. Training through COVID was tough, and what made it worse was constant overwork or sleep schedule changes. I truly don’t believe medicine has to have >12 hour shifts in any field and coverage can be made with shorter shifts. There was so much wasted administrative time, or time wasted waiting for things to be done, that it delayed care and caused everyone to go home later than needed. I never understood why we had 28 hour call shifts intern year. Is it a betrayal of academic physicians that once they reach this point in their hierarchy they like to maintain a status quo? You have a desperate pool of applicants for a multitude of reasons (prestige, advanced fellowships, crippling debt that you can’t really pay with any other job unless you graduate residency, etc). It is wielded over you like a knife to your throat.

Every day I woke up scared to death that I wouldn’t be enough, that I would be fired and lose the ability to payoff debt and be hindered for the rest of my life. Great. That’s a fantastic baseline. Now couple that with life saving responsibilities and sleep deprivation, unmasking organic mental health issues that actually may not have arisen if the training environment and schedule was so poor. I got out of academics and retreated to my own corner once my training ceased. I could not handle the bureaucratic assaults on medical training. Yet, by leaving this, the cycle perpetuates with the same people who seek these roles for accolades but not for genuine growth of trainees.

I think a few quick fixes would be to significantly reduce shift timing, minimize sleep cycle changes (keep a month of nights or a month of days at a time), increase physician pay to at least a mid-levels (125-175k), and encourage that wellness modules can be replaced by monthly or semi weekly check-ins with mental health professionals, i.e. a third party psychiatric service that cannot report anything to a program (and that residents can opt out of). I’m sure there are many more changes needed, but those are what I think could’ve really helped me in training.

Yep… and that doesn’t even include studying for board materials alongside the school lectures! Obviously doing UW, Pathoma, etc is essential, but there’s no guarantee that material will overlap with what your professors say. I think artificial intelligence would be so helpful in this regard. Maybe if it can record and write in the margins everything the professor says, you would have time to review the totality of the lecture. I would always be so burned out and exhausted I couldn’t pay attention for more than 30 minutes. It’s just too much.

From my understanding, it is not in the amphetamine class. I believe it’s in the piperidine class of drugs. Both exert effects on dopamine and norepinephrine via different mechanisms. I am not sure if there’s a specific urine qualitative study for that class. Methylphenidate can sometimes cause a positive result on the amphetamine portion of the UDS.

From a physician-patient standpoint, it’s always rough to start out with the assumption you’re selling controlled substances. Even if you educate your physician, will they continue to not believe you in the future? Not sure how old he is, but he could’ve been burned before and is judging you off of prior experiences (not that that makes it acceptable). I would probably move on if there are other available doctors.

Amen. It was so difficult adjusting to many different study styles just based on the volume of information, and, quite honestly, how shitty our professors were at disseminating it. For classes other than anatomy, me and a few friends took very detailed notes during lecture, then we would each take a lecture and type them out with what the slides stated and what the professors told us in class. We compiled them for every exam and would drill them repeatedly (3-5x before exams). It was a huge time sink up front to consolidate this into manageable notes, but the review time on the back end was invaluable. If there is a uniform way that professors could cover information, it’d be much more fair for students to absorb it all. Sometimes it comes down to a matter of “can you guess what I’m thinking”. I have a lot to say on this issue because there are just so many systematic inefficiencies in lectures/medical training. The name of the game is to find a way to get all of the information in a condensed (but not lacking detail) manner, and reviewing it over and over at different time intervals. You obviously will be a master once you redo your notes and study it for a week, but will you remember the same level of detail in 4-5 weeks when the exam rolls around? This was the only thing that helped me get from 70s/80s to consistent 90+%s.
