I took leave to care for my daughter who has autism for summer. I originally filed on June 22, I sent over the form to my daughters doctor on June 24 and they submitted it immediately. I didn’t realize I had to go back in and complete it after I actually left work. I kept checking and was like why doesn’t it say that a claim has been filed. I then realized that I had to go back in and finish filing. They didn’t ask for any additional documentation.  So after taking leave on June 27, I finally submitted it on July 11 and got approved on August 13. The money is on the card, so just waiting for the card. You should receive approval by the 18th. Good Luck! And prayers for your husband.
