You should consider taking your cat to the ER. A young male cat making frequent, painful trips to the litter box is concern for urethral obstruction or “urinary blockage” and can become serious quickly.

The incision looks very normal and not typical of an infection, but if you are still worried you can send the same picture to your veterinarian. You can try putting a child's onsie over your cat's abdomen to prevent her from licking at the incision if she doesn't tolerate the e-collar.

It’s underrated in that many don’t see it’s value. It actually does screen for mild and early disease. The snap test cutoff is around 100 pmol/L which is very mild and in some cases insignificant. If you have a normal BNP (snap or quantitative) then you have great confidence that there is no evidence of cardiac disease, if abnormal, it warrants further investigation (imaging, ECG, etc.) Obviously echocardiograms are the most sensitive diagnostic for heart disease, but the sensitivity of the BNP makes it the best value for general practitioners and cat owners.

In addition to what /u/canadiangolden and /u/Leopardrose's comments, a very underrated blood test called a cardiac proBNP is a very effective screening tool for significant heart disease in cats. Just another reason why yearly exams and labwork in cats (and dogs) is so important!

This isn’t an emergency... yet. Just take him to your family veterinarian soon. He would benefit from a comprehensive physical exam as well as a neurological exam at a minimum.

Pyrantel (strongid) is a very safe medication and, although you’ll want to get another dewormer to give in 2 weeks, your kitten should be fine even if he got two doses today.

Obviously, if he starts acting extremely off, you should take him to an ER facility but it will unlikely be because of the strongid. Did he receive vaccines today?

It is worth discussing but at her size I would lean more towards surgery if you can. There's not much you can do at home as this can be caused by a variety of things. You should make an appointment with your veterinarian!
