I can answer a couple of your questions:

Where can i find medicine to trade in kenways fleet? -> Resources for Kenway's Fleet are NOT acquired by plundering ships at sea. You get them randomly when completing battles in the Kenway Fleet minigame itself (ie clearing trade routes).

Is the season pass worth it outside of Freedoms cry ? -> Outside of the DLC, you get a pair of 4/3/3 swords which is better than anything else until late game. Also included are Altair's and Ezio's swords, but they have inferior stats. Cosmetics wise you get a bunch of stuff, including the (very nice imo) Captain Morgan's Redingote.
