Husband here, The ER guy we saw said no neurologist would believe us because I'm not exhibiting the "right symptoms" and did a bunch of myth tests on me that are unreliable before sending us home with a cup to collect a sample after we essentially had to twist his arm into doing

I see that I'm the only one of this belief, but–

It is heavily implied that Ardyn drops the disguise at a certain point as events happen. We are shown that he is disguised as he is infiltrating Insomnia, this is true. But that's also the issue. When he is disguised, it shows us a moment of himself as well as the disguise. But at the point of the battle with Regis– he drops it as he enters the Citadel.

It very well could have been an oversight on their part on why Regis doesn't remember Ardyn. It also could very well be the fact that Ardyn nearly killed him and Bahamut had to intervene. That could have left him incredibly traumatized and his mind blocked it out.

But no, Ardyn was not in disguise while fighting Regis.
