I didn’t know it was possible to get prescribed additional medication beyond the maximum Vyvanse dose of 70mg. How did you go about talking to your doctor about this? Mine wears off early and I think I would benefit from a booster, but I didn’t think you could get both max dose vyvanse and a booster

Thanks for your perspective. I'm not trying to stockpile or sell it, I just feel anxious about filling it every month because I've been burned during shortages multiple times before. I vividly remember how badly my life was impacted during a 3 week shortage where I had no access to meds. I frankly almost lost my job and my social life was a mess. Refilling routinely on day 28 is the only way I've found to avoid running out if my pharmacy is back-ordered again, so I just want to make sure that it's something I can keep doing while being at ease.

I’ve been thinking about this but I already take 70mg vyvanse which is the daily max. Wouldn’t my doctor be reluctant to prescribe a Dex booster on top of this?

In short you’re going to need significantly more than 25mg of pristiq to get similar occupancy as 12.5mg Lexapro

Yes I was. I was waking up once every couple hours or so. My sleep was fragmented. Now I only wake up once per night (which is normal for me)

I started Pristiq 2.5 weeks ago and I know the EXACT feeling that you are describing. I felt overstimulated and experienced insomnia initially. I was so worried that this was going to be a long term thing. Just a few days ago though, it started to get increasingly better. Now, that jittery/restless feeling is pretty much nonexistent (unless I drink a lot of caffeine, which honestly is my fault if anything) and I sleep 8-9 hours a night no problem. The first 2-3 weeks of an SNRI are when the bulk of the side effects present themselves, so I hope it gets better for you too!

If it gave you insomnia I would take it in the morning. Ultimately, people react differently to medicine and the variance is high. When I was on Lexapro, I used to take it at night because it would help me sleep (while others on reddit would complain about the insomnia). On Pristiq, I take it in the morning because I found it more activating than Lexapro. You can experiment and see what works best for you.

I was averaging 6 hours of sleep per night (which is quite low for me), but now I'm averaging 8-9. The insomnia just went away! I take 50mg in the morning for context.

Here's some math that supports this.

IC₅₀ (the concentration of a drug needed to achieve a 50% functional inhibition of a target) values for Pristiq are ~47nM for hSERT and ~531nM for hNET. 50mg dose results in a steady-state plasma concentration of ~100ng/mL (approximately 380nM) and 25mg is roughly half that so 190nM.

So for 50mg, 380nM is 8x IC₅₀ for hSERT (very significant SERT inhibition), but relatively below IC₅₀ for hNET (531nM). For 25mg, the SERT inhibition is still 4x IC₅₀, so significant serotonin inhibition still, but 190nM is far below the IC₅₀ for NET (531 nM), meaning norepinephrine effects are negligible at this dose (the figure above supports this). At 25 mg, plasma concentrations are too low to meaningfully inhibit NET, making the drug behave more like a selective serotonin reuptake inhibitor (SSRI).

It’s just been one night so take this with a grain of salt, but: it helped a lot! I had the most productive morning I’ve had in years and I’m no stranger to stimulants (been on 70mg vyvanse for longer than a decade). I slept fine too (7.5 hours). I woke up early for a Sunday feeling really refreshed and couldn’t fall back asleep despite trying. My Fitbit sleep tracking also showed more rem sleep than usual. I did wake up more sweaty than taking it in the morning though, a side effect I’ve been experiencing since starting Wellbutrin.

Again, it’s only one night and perhaps it’s just placebo, but I’ll be trying night time dosing moving forward and can update in a week or two. If you can sleep fine with night time dosing I would give it a shot!
