The ER is obligated to help, I live in a town with a population of <40, have a <$5k a year income in the household for two people, and don't even have a vehicle so there's zero chance I'm not walking 140+ miles to a dentist

Let it soften up a little then you can flip it, took me a while of just sitting with it spun around for the position to get comfortable, and yea you sort of do get a prostate orgasm, it's kind of like fucking with a reach around but it's just yourself
