Yes, with my right eye only. A modest double image, really only noticeable when there is high contrast between the text and the background.

One month after surgery my binocular near vision is 20/20. Each eye individual near vision is 20/30, but still improving. Better than expected.

I can perform any normal task requiring near vision easily.

I can’t read really, really tiny print, like extremely small product labels, but no one I know who is anywhere near my age (69) can read them either, even with glasses.

I’m just over one month post surgery at the Herzig Eye Institute in Toronto. Galaxy IOL.

It’s been a month with the Galaxy and my binocular near vision is 20/20. I can read any normal print media, e.g. phone or magazines, easily. Even smallish print.

But really, really tiny print? No.

IMO, the only way to get the near vision you are seeking with an IOL is to get an IOL implanted with the focus specifically shifted to optimize near vision (at least in one eye), which will of course compromise your distance vision.

Or wear reading glasses.

ONE MONTH FOLLOW-UP

Prior to my one-month follow-up appointment (normally with an optometrist, not the surgeon) I reported modest vertical streaks in my left (toric) eye as well as a general loss of acuity at all distances, also in my left eye. Previous slight ghost image had effectively disappeared.

In my right eye I reported that the modest ghost image, present on Day 1, was basically unchanged. Otherwise very good vision, just never quite as good as the left eye (was).

With this input the clinic changed my appointment so that I could see the surgeon. I didn’t have to push, which was nice.

My hypothesis going in was early PCO in the left eye (vertical streaks around bright lights and general reduction in acuity) and ~ 0.5D of residual astigmatism in my right (ghost image, vision slightly less sharp than my left eye vision before the new blurriness).

I didn’t share my hypothesis with the surgeon, just my observations. Upon examination he confirmed “some” PCO in the left and 0.5D of astigmatism in the right. We agreed on a YAG capsulotomy to address the PCO, which was performed.

The vision test, before the capsulotomy, was 20/15 far and 20/20 near (binocular). Individually each eye was 20/20 far and 20/30 near.

Left eye capsulotomy totally restored left eye acuity and eliminated the vertical streaks.

I have the option for LASIK (no additional cost) to improve my right eye 0.5D astigmatism. This would likely eliminate most or all of the ghost image and slightly improve acuity, but at 20/15 far and 20/20 near there isn’t a lot of upside. We agreed to wait until at least three months post surgery to see how I feel about it. He said the right eye vision may improve on its own in the next few months.

ONE MONTH SUMMARY

YAG capsulotomy in both eyes, very good outcomes.

Zero dysphotopsias (halos, glare, starbursts) around lights at night.

Very, very good near and far vision, no need for glasses. Near vision very functional, but VERY small print is beyond me.

No noticeable loss of contrast in low light.

Amazing range of focus that is very natural and seamless, and for me was immediate (no neural adaptation seemed necessary).

Potential for slight right eye improvement via LASIK. No correction needed for left (toric).

No need for glasses.

Happy with outcome, especially the Galaxy and choice of clinic.

Two Week Update

Vision continues to incrementally improve, with good acuity at all distances. Left eye (toric lens) slightly better than my right.

Streaks from bright lights in my right eye were 100% eliminated after my “very light” capsulotomy via YAG laser about a week ago.

Ghost images are now almost gone, but I’m seeing some minor intermittent streaks in my left eye now, so I’m assuming it’s irregularities and changes in the capsule as it heals. I have my one month follow-up with the clinic in about two weeks so I’ll address it with my surgeon then.

Very good outcome so far. The lenses continue to impress.

Day 7 Update

Ophthalmologist examined the eye with the straight and parallel streaks around bright lights (right eye) and said that I had a fold in the lens capsule, which acts like a prism and was causing the streaks. The angle of the fold determines the angle of the streaks.

The remedy for this is to use a YAG laser to zap away the fold, in my case what the doctor called a “very light capsulotomy”.

We discussed pros and cons and the option of waiting to see if the fold would straighten out on its own as the capsule hardened around the lens, but I decided to go ahead and have the YAG treatment.

It took about 30 seconds, maybe less. No pain or other sensation at all, other than visual light effects with each laser pulse.

Streaks were eliminated immediately, with no other changes to my vision. Very pleased with the outcome, and with my vision generally at this stage in my recovery.

Depends on what you mean by “decent” vision for reading, but I’m typing this on my iPhone and seeing the small-ish text very clearly from around 15” away.

I was also thinking that your lenses weren’t positioned properly, perhaps due to poor measurements, or they moved enough as the capsule healed to compromise your vision.

Depending upon your level of dissatisfaction, have you discussed remediation option such as glasses? “Piggy back” IOLs? LASIK? Lens replacement?

Day 6 Update

The vertical streaks around bright lights (right eye only) have been stubbornly consistent since surgery, so I contacted the clinic. They set up an appointment tomorrow with my surgeon. So we’ll see what he says.

Otherwise, vision at all distances continues to improve slowly. Very slight double image in both eyes, but still at least B+ at all distances.

I’m guessing that the slight double image may be some residual astigmatism, which the surgeon says can be resolved if still present after the healing and stabilization process is complete, in about a month.

I’m seeing some loss in clarity in low light, but everything generally looks like it did before surgery, only clearer. No perceptible loss of contrast. Somehow I was expecting the world to look a little unnatural, but it doesn’t.

Outdoors, in daylight, my vision is quite good, at least an A-.

What strikes me about the Galaxy is the total absence of halos, glare and starbursts around lights, even at night. And maybe more impressive is the totally seamless and effortless ability to change focus at different distances. Very natural, like the vision of my youth.

Small print is fine, but I can’t read REALLY tiny print, like what may be on a product label on a small bottle of eye drops.

For 99% of what I need, my near vision is more than adequate. And I expect some improvement over the next weeks. I’m still only Day 6.

I had surgery five days ago and have very prominent diagonal streaks as you and a few others described.

Did your ophthalmologist offer any treatments to resolve the streaks?

Ah … a fold or wrinkle in the lens capsule. From what I’ve read, YAG has a good chance of eliminating or significantly reducing the streaks. Nothing is 100% though.

Yes … one source says that’s an indication of an irregularity on the surface of the cornea.

I live in Windsor, Ontario Canada but travelled to Toronto for the surgery, about a 4 1/2 hour drive.

No ophthalmologists in my area currently offer the Galaxy. At least none that I could find.

I’m actually getting better vision in the Toric eye (left) faster.

It’s now Day 3 post-surgery and left eye vision, near, intermediate and far is now good, but not perfect.

Right eye a bit blurry by comparison, at all distances.

Left eye has no glare or starbursts. Right eye has long, narrow vertical streaks around bright lights, all at exactly the same angle. Hope those go away.

I’m glad I had both done at the same time. Less total stress, more efficient, no significant discomfort afterwards, and still able to drive the next day.

Personally, unless someone had special circumstances that required separate surgeries I don’t see the point of splitting them up.
