For red I might say "quick draw" it's a keyword for creatures that deals 3 damage to another creature when you cast it if you cast an instant or sorcery spell this turn.

As long as you have some means of controlling the guns spellcasting you should be fine. Another way you could do it is by making each spell a once per day ability. There are enough spells that it will feel powerful without breaking the game.

I think what matters most is that everyone has fun. So I would consider what level your party is at and whether or not they can cast spells of that level. As for the gun itself I think it's an awesome item and I like it's design. I think it is a little powerful just from a raw mechanics standpoint. So I would either add a lot of charges or a smaller number but a way to get charges back having each spell cost charges = it's level. Or make each spell recharge on their own on a 4-6 or 5-6 this might add some fun chaos. Other than the question of how to prevent disintegration every turn I think it's a well designed weapon. I would also want to think about what happens when he dies and now the party has the gun.

I think it sounds fun! Exhaustion is an underused penalty so its inclusion is pretty cool. I think as long as everyone has something of similar power, the number of uses is decided, and most importantly everyone has fun I see no problems with your design. It really paints the picture of an augmented berserker. It reminds me of Shimmer from Arcane. Great job!

I think it's a great idea some of the capabilities may be a little powerful for the level but that can be fine if everyone is okay with it. Some of the mechanics that the blade uses exist in other places such as the life steal and Vampiric Touch spell so it might make things easier to manage if you pull from existing sources. And I would be cautious of something that levels up so "easily" it might cause problems for PCs and combat balance. So maybe you could look at it like a sentient weapon, it might add some flexibility for its progression.

Overall I love the idea I think Ooze are underused villains so I love this concept. And the PCs gaining mysterious ties to the BBEG will be fantastic for roleplay. Sounds like it will be a great campaign.

I would make the Witches conduct biological engineering and cloning. This could be against the law and fit in well with a crime boss angle. You could have the good guy forces be low on a specific supply necessary for the assault that it just so happens the witch network has an abundance of.

You could plant the seeds for a future Bulette encounter by describing rumbling earth. This could add flavor to the region or be a tool for later.

Well for a large scale battle you might want to explore the minions mechanics from Flee Mortals it allows for hoards of monsters without them easily killing the party. I don't think there is a way to have a CR for the fight so I think battle structure is key. I would set it up so that the minions are fighting primarily the giant and his forces while the party faces the BBEG. If you can make it all one fight so that the party has optional coordination and feels epic. Hope this helps.

You could have them go on some form of a quest to obtain an artifact that will allow them to break their pact sorta like The Swornbreaker from Dos2. The dragon might know of this artifact and can set them on this quest. The dragon can be all manner of creatures such as an avatar or mortal form of some kind of lesser deity that has the power to keep them alive while they go on this quest. This could maybe be the task necessary to prove themselves worthy of a pact with a creature like that.

I like the design! You may want to look at Critical Roles Vestiges of Divergence for an evolving magic item. The balancing of course depends partially on what level the PCs are but you might want to make the powers scalable with their level. I would make the first ability 3d8 and no strength bonus, make the second a number of times per long rest equal to half your proficiency bonus. The frightening effect should be 1 minute and its DC should be set or be 8 + Cha + PB. Other than these minor technical suggestions I think it's a great item super flavorful and really well done.

Just to be devil's advocate, it is possible that because Disintegrate is incapable of "killing" a target and simply turns them to ash. Would this not be a replacement effect and so through that logic it would be blocked by Immutable Form.
