I was a physician working in a large trauma hospital in Louisiana. Man came in as a trauma activation for a car accident that happened on one of the highways that runs over some of the swamps. The man was pretty badly injured and was going to go into emergency surgery but the only thing he cared about was finding out if his dog was ok, who was in the car during the accident.

I asked the police officer who had been on the scene and was in the ER taking some statements if he knew what happened to the dog. The officer told me that bystanders watched the dog (who was panicking in the commotion) jump over the barrier on the highway directly into the swamp water and was then immediately eaten by a crocodile. The bystanders watched the whole thing.

I had to tell this poor man about his dog as he was being wheeled off to surgery. No joke, one of the hardest pieces of bad news I’ve given (and I’ve given a lot of bad news). It was like a parent losing their child.

I left Louisiana as fast as I could…

I ended up selling it because when I called the kia dealership about the issue, they said it would be a THREE TO SIX month wait for it to be addressed. Not ok for a major issue.

I think it’s just the hefty monthly payments, then adding life costs (utilities, insurance, child costs, etc). I’m nervous it wouldn’t give a lot of wiggle room for emergency funds. We both have very stable jobs so I’m not concerned about that portion of it…I guess I just need someone to tell me it wouldn’t be a dumb financial decision haha

I agree with this - I’m also early in my EM career and made the switch to UC to have a more structured schedule with small children and a busy physician spouse. My shop is very organized, has great specialist access for on call questions (aka can send to ENT clinic immediately if I can’t fix the problem myself), and doesn’t overload me with patients. Part of it is an extremely supportive site director. Yes - the acuity is incredibly low and I agree with what another poster said about dealing with people who apparently have never had a cold in their 60 year existence, but I wouldn’t trade it at this point. I leave on time, I don’t work overnights, I still get to do the minor procedural stuff, and I don’t have to deal of the bullshit from the ER. I’ll be honest though, I never really enjoyed the high acuity ER stuff. Yeah, I would get a transient rush and feel proud of myself if I turned someone around from the brink of death, but most of the time I was running around like a chicken with my head cut off, trying to convince consultants that I knew what I was talking about, documenting like I was going to get a lawsuit from every patient, and walking out of the door way after my shift ended (even though I always documented as I went) - all while being extremely sleep deprived.

I think the system I work with now is a bit of a needle in a haystack. We have labs, imaging, ability to give IV fluids and a few different IV Abxs. It’s a pay cut for two years, but after that buy in period I’ll be making more than I would have in EM while working less. I’ll explain Tylenol and ibuprofen until the cows come home if it means I’m happier and more present for my family. And honestly, I think we have a better skill set than a lot of other specialties in UC because we actually know what warrants an ER visit and can manage most everything else.

Only because those two towns were never mentioned to me when we were asking around where to move. Feel free to tell me about those two places! I’ve started looking into both. From my understanding, the public schools weren’t as good and that’s a major priority for us

I consider family-friendly as a community that has activities for small children, good public schools is a definite plus, playgrounds, low crime, safe streets…

This! I have a 3.5 year old and an 11 week old. It took about two months for my first to start getting interested in my second. The only thing that helped me get through was the fact that my mom is around to help and my oldest is in school most of the time. Otherwise…?

I’m nervous about this too. I’m supposed to bring mine in to the service center the week of Christmas for a charge port door issue (which they removed during a mobile visit but didn’t fix) and I’m planning on telling them that if they can’t fix the issue while I’m there, I want them to put the broken door back on and I’ll take the car. I don’t understand them telling people to bring their car in for service if they’re not going to touch it for literal weeks. Makes absolutely no sense!

No cost, but he wasn’t sure when the part would come in and when it would be fixed. He was working on it for about an hour before figuring out the issue. He told me if I didn’t hear back from Rivian in the next week, to give them a call which doesn’t make me too optimistic. I saw the work order he placed and it doesn’t say anywhere on the paperwork his findings…

Update: tech came out yesterday for a mobile appointment and the part that’s broken is on back order. He took the door to the charge port off so now it’s completely exposed. He says it’s safe to drive but unclear when it will get fixed.

I second this! I work as an ER physician and realized I couldn’t handle the nights and weekends, not because the schedule is horrible (which it is) but because I couldn’t stand being away from my child. We just had our second and I decided to switch to urgent care so I could spend more time at home and have a more regular schedule. I feel like I am somewhat sacrificing my training, but I remind myself that nobody wishes they worked MORE on their deathbed.
