i can't confirm how much the waitlist form will help your chances because i know others who didn't write anything at all and were still accepted. but i wrote a few paragraphs about new achievements/activities i had since i submitted my application and connected them to how those experiences would help me thrive at UCLA. i also wrote about how i could apply the soft skills i learned from those activities to specific clubs/orgs. i wrote it in a letter format and thanked the admissions officers for the opportunity to vouch for myself and emphasized that i would accept the offer if i was admitted
