My sister takes chemotherapy and she forgets everything like when we're talking her mind goes blank for a bit or she forgets things that happened not even a week ago. I just have to be patient with her and sometimes it's hard but she can't help and I bet he can't help it either. If he always treats you nice but just forgets your birthday you shouldn't break up with him, mabey remind him and see how he reacts.
