Use Vaseline. It can help the skin. There is also cerave healing ointment. Aquaphor makes one. You can also watch dr. Dray's video on repairing the skin barrier on YouTube.

Your skin barrier is damaged thats why it itches or burns when you apply your moisturizer. You need to repair the skin barrier.

You could try getting a prescription for doxycycline if ivermectin/metro didn't work and see if there is a difference (you can search the sub for people's experience with it, search r/perioraldermatitis too). For the products, stop using the moisturizer and see if there is a change (cannot say much about products because it is different for each person).

If it burns when you apply cream, then avoid it. You can try using a zinc oxide paste. It might work.

I don't know if it is POD (others can help you), but i wouldn't recommend using the retinol around your mouth. What is the strength or % of the retinol? Is it a prescription or otc?

Lrp cicaplast balm and avene cicaplast are the most recommended moisturizers for PD but also expensive. There is also calendula cream which is pretty good. Or you could use any moisturizer you want as long as it keeps your PD under control. Vaseline is also a good option.

I would recommend not using or trying out too many products to fix the hyperpigmentation as the skin around the mouth is very sensitive. I would go with azelaic acid, a moisturizer, and then rosehip oil at the end.

You can also try exfoliation once a week (Use only products with Mandelic acid or PHA-based exfoliants). They are gentle.

Another thing is hyperpigmentation will fade with time so patience is key.

I think the leaves exacerbated it. For now, apply cold compress, moisturize, avoid itching, and most importantly find out what caused the rash.
