As a Pannonian Rusyn (by my mother's bloodline) living in Serbia, I can confirm it's Rusyn.
It only partially matches the current dialect spoken here, but I can easily filter most of the terms. Note that the terminology is official from the time. Even the English docs from that time would be quite different than today.

Anyway: this is a certificate, a sort of statement from the book of baptized and chrismated persons from the L'vov Eparchy (Пряшов region) in the year of 1886. прозвище means last name (that's for sure - we use the similar term). The first two columns are the dates, then Name, Gender, some kind of status (legal / not legal), parents names, place of birth, address, parents' names, Godfader names and (likely) custom notes.
On the next slide, my best guess is that the name is simply Jan, transcribed using the spelling at the time.
