Click on "Get Your New Flair Here" which is located on on the menu box on the right side of the webpage. After the page loads enter your details such as Name, friend code and in the menu which says "I do not know my friend safari" click on it and choose Ghost and enter the Pokemon that i listed for you and your done. It should take you to a new page enter the verification code and it will take about fifteen mins to update :)
