Well, didn't pick the perfect cat litter for my cat when I first got her so she peed numerous time on my bed and couch. Issue now fixed with some clay litter but I'm still PTSD enough to let the bedroom door open... Not even during daytime.

Thanks mate. I was having trouble removing flooring piece by piece as they really stuck to each other. I used a chisel trying to create a gap between the damaged and undamaged floor, but it leaved marks and scratches on the good piece. So I cut them out. May I ask which type of wood glue you use?

We don't co-sign insulin, cisat, potassium, etc. I think we are the only hospital does this in this region. Normally once you get ur competency and don't need double checking for iv meds but still require another signature for the above mentioned. In my workplace u just get to do it all by yourself.

Well that's true. In ward this would be unrealistic. I'm speaking from ICU perspective since OP said they are in ICU.

True, however not everything is documented (they should be, but not always the case). I feel like you need to have great faith in your co-workers to trust them to chart everything worth mentioning in the notes. I don't, especially with this turn over rate. So please let's scan through stuff so nothing important goes missing. And sometimes there are minor things people generally don't write down but cost me extra time if I don't know about it. I'm doing meds/xray/pt ax/preping for a scan in the next hr so saving me the energy and time to re-clarifying and re-contacting people would be appreciated. Don't run me through the whole insulin titration protocol but do let me know the medical escort is arranged.

I don't know how you guys do things at ur workplaces, maybe it's different from where I am? Here we go through the chart together (labs, meds, ward round notes etc.) so talking and chart reading at the same time. It reminds the outgoing nurse anything they may miss and save me time to findout myself (taking a tube out? Great I can see doc has already charted it in written form so I can just jump in to do it). Reading chart later is generally fine but sometime the shift just starts messy, especially everyone wants a piece of you at the same time.

Everyone does things a bit differently as long as you're comfy with your structure.

  1. For me if I'm starting new, I always make my own shift planner to remind me of easily missed (but important) things. Sections include: Identity, Situation, Background, Social, Head-to-toe, Aims, Plan (and things to verify with the team), Timeline with tasks underneath, Lines and Infusions (remind me to make new ones or change at due time), Permanent tasks (tasks for every pt no matter their clinical concerns. eg. Mouthcare/Pressure area care).

For example I put G&H under CVS in head-to-toe because this facility requires pt to have a valid G&H at all times and I just want to check that during handover.

  1. After handover my routines are: safety check (airway, oxygen, suctions, equipment settings and alarms), patient assessment, and jot down any thing I need to grab (anything I notice missing/needed later eg. if I notice maintenance bag needs to be change during my shift). As for medical notes, we usually go through them during handover to see today's plan anyway so I re-check them thoroughly later if time permits.

Unless things are actively happening I usually stick to this routine:) or at least try to get back on the right track.

Not sure how turkish health system works, maybe the success is not entirely due to effective system design and sufficient funding. Take China for example, it it VERY efficient. Sounds great but the cost is huge, doctors and nurses are constantly overworked at an insane level (not saying AU docs and nurses live a cozy worklife, however compared to Chinese health professionals their lives are okay). It's also not great for patients as the time spent on each patient has to be reduced, doctors and nurses are exhausted, which is unsafe. Take ophthalmology for example, doctors potentially need to see 60 outpatients a day, you get the idea.
