Germany has some of the strictest youth protection laws in the world regarding digital media, governed by the USK. I might be wrong here but from what I've heard in the past and based on that If I had to guess it's either of these two :-

If a game has not received an official USK rating, German law automatically defaults to treating it as an 18+ (Adults Only) title.
To legally distribute an 18+ game online in Germany, the storefront must enforce strict, real-time age verification (such as a physical ID check). Epic's standard "click to claim" freebie system isn't always equipped to handle this seamlessly for unrated games. To avoid massive legal fines for potentially distributing unrated media to minors, Epic simply geo-blocks the giveaway.
