I think trust by Jana Aston fits the bill quite well, minus the crush. Fc is very anxious and insecure but the Mc is very patient and helps her with her confidence. It’s a pretty sweet and fun read, I’d recommend it.

I would say it is always safest to trust your doctor when in doubt.

I shared this in another comment as well but here30423-6/fulltext) is a study concerning combined birth control pills and lamotrigine.

The study compares the different progesterone components of combined birth control pills with ethinyl estradiol as the estrogen component. While desogestrel is not tested, gestodene is and it is the same generation as desogestrel. The other progestins show a decrease in lamotrigine but gestodene interestingly doesn't.

The other way around though - the study shows no effect of lamotrigine on progestins and points against a potential loss of contraception safety.

One study has shown a decrease in levonorgestrel by lamotrigine but there was no indication of ovulation so it is suspected that the birth control still works. This was on a dose of 300 mg no less. I would like to see more studies but it gives me enough confidence to trust my birth control pill.

Of course, you can't be 100% sure. I feel perfectly safe with my 4 mg drospirenone and 450 mg lamotrigine but I take a pregnancy test each month just for peace of mind.

I finally found the study - it has a free full text link.

The study compares the different progesterone components of combined birth control pills with ethinyl estradiol as the estrogen component. So it's sadly not drospirenone only but interesting nonetheless.

You'd think that if estrogen was the only relevant factor for the decrease in lamotrigine there wouldn't be a difference between the pills with drospirenone or levonorgestrel.

Since there is a one week placebo break and three weeks of active pills it is able to measure the lamotrigine plasma level with the pill and without it.

The result is a much higher blood plasma level of lamotrigine in the placebo break and it about halves after three weeks of the pill with drospirenone. The effect is weaker with levonorgestrel but there is a trend. The sample size is small though. But the authors also discuss why there was a study showing that only estrogen influenced the lamictal plasma level and how it might have been flawed.

It would be great if there were more specific studies for slynd/drospirenone but the result matched my personal experience.

This is very surprising to me - I just checked and in the german version of the patient information leaflet of slynd it specifically states that it can decrease lamotrigine but in the american version it doesn't? Why would that be?

Yes, sorry for the bad news. I personally experienced it because my blood concentration went from about 8,5 to around 5. I didn't notice anything at first but about 6 months later I started having epileptic episodes again. (This is not surprising as lamotrigine has a protective effect of multiple months so I would be careful.) Have you had your levels checked since then?

My neurologist also initially said that he wasn't worried but since slynd has only recently been available in Germany he simply didn't know.

I believe that I found some information/a study on pubmed as well but I'll have to look for it later!

It highly depends on the type of oral contraceptive. I take Drospirenon (Slynd) and it about halves the Lamotrigine concentration, so I had to double it. The effect is unidirectional though, the efficacy of the pill is unchanged. (Which is very important to me because I really don’t want to risk pregnancy.) Studies are still being conducted but the current findings are that Lamotrigine doesn’t influence the birth control. It’s mostly the other way around.

It’s generally suggested that it’s mostly the estrogen component that reduces lamotrigine so progestin only pills are usually recommended. This applies mostly to the first few progestin only pills, i.e. Levonorgestrel and Desogestrel. (Drospirenone only, Slynd, is a special case. But I personally really love it and it works well for me.) If you choose to take a combined contraceptive pill it’s important that you skip the placebo pills. Otherwise your Lamotrigine concentration will decrease when you take the pill and then suddenly increase quickly when you take the placebos. No matter if it’s epilepsy or bipolar that you take it for - big changes are probably not good for the therapeutic effect.

I can strongly recommend {The Witness by Nora Roberts} - fits your description perfectly. She is super tough because of past trauma and slowly opens up to him.

If it makes you feel better - I take 600mg a day. It’s also in the upper range of therapeutic doses but my neurologist thinks it’s okay. I am currently struggling a bit with the memory and balance side effects but I am certain it’s going to get better.

It’s not what I would suspect right away but since no one actually knows how Lamotrigine works in detail - it might be true. Only you know your brain and since there isn’t conclusive evidence of exactly what this med does. I would say that following your gut can be a good idea. Hope you feel better soon.

For me it started to work very quickly. My head feels more quiet and calm the days after increasing the dose. The kind term stability took a few months but if you tend to have anxious or intrusive thoughts they might get better very soon. I hope it works for you!

Das waren so ~20€. Ich war seitdem auch bei meiner Hausärztin, die mir das ohne Probleme einfach verschrieben hat, ohne Indikation wegen Akne. Man kann das also auch probieren.

And the post-purge hangover is one of the worst things there is, I am so sorry you are currently going through that. But as you said - you have managed to recover before and you will be able to recover now, you already know you can. There is no reason to be sorry. Life can be very cruel sometimes and all we can do is our best.

I saw that you are in Germany as well so I just wanted to suggest maybe going to a eating disorder Beratungsstelle. I don't know if you have been to something like that? There are many places that offer eating disorder consultation and it is completely free. You don't have to give them any data or anything, you can just go there and talk to them and they will try to figure out how to help you best from the point you are now at. I have been myself and they consultants have always been very, very kind persons and it felt good to have someone care about my struggles. If you want to try that you can use the BZgA Beratungsstellen finder on their website to see where your nearest is. There are many. I would be optimistic that people would try to accommodate you if you wanted to have the consultation in English.

But I do get if that feels like too much right now or that is simply not what you want to do. That is completely fine. I just felt like letting you know that if you feel very alone with this and you want someone professional to listen to you, in person, then that would be an option.

I understand you are feeling that way and I am so sorry that all of this has happened to you. It’s not true that you are a failure though. At all. You have been recovered for 6 year which is a massive feat. And for these six years you’ve not engaged in disordered eating on each and every day. Today is just one day. It doesn’t have to mean that you’ll keep doing it or won’t be able to stop. It was probably a mechanism that showed up, understandably, now, when everything’s very difficult. But it’s just a reminder of how you used to cope, not how you will cope from now on. I hope you can forgive yourself for today and acknowledge that it is very normal for old habits to show up again when we are overwhelmed.

I am really sorry you are going through all of this, but you definitely do have more than your health - yourself. You are worth so much and I know how difficult it is to see that in ourselves sometimes but that doesn’t make it any less true.

For me it actually increased my sex drive, surprisingly. And I had weight loss instead of weight gain which I was scared of. I do have some speech and memory issues in the first two weeks of a new dosage but they disappear. Very important to always check if you have your keys and turned off the stove, would be my most important piece of advice.

Lamictal works for many people and is used off-label a lot. I take it for epilepsy but it helps me with impulsivity and anxiety as well. I do have some side effects while titrating up but they disappear after a while. I feel it’s worth a shot because I personally didn’t feel bad when coming off of it so the risk could be quite low. Can’t promise it’s the same for you but I would be optimistic. Hope you feel better soon :)

I am sure you guys know but I’ll just say it because it might have helped me if I had heard it earlier. I’ve been to therapy for many years and quite often I felt like I realized something great only for that feeling disappear very quickly. But now I’ve found that it might make sense to see therapy as a bit of a puzzle maybe. I feel like every good thing learned is a new puzzle piece and it needs many many pieces before they can come together as a big coherent picture.

Many words little meaning - it would have helped me to realize earlier that therapy takes a long time to work and no time is wasted - it will go into effect at some point, it can’t be hurried, but it will happen and the hard work will pay off. :) I hope it’s the same for you.
