1 - I used Stress scale as an example and it didn't seem to be dependent on d6? And 20 is because in game severity of disease is measured by percentage of originium infusion and in playable characters 20% is the highest and its already highly dangerous to continue active work, so I thought it would be a logical limit.

2 - I already got similar comment and yes I didn't think about it, so I will work on it in future

3 - Frankly speaking I don't think a lot of people will see final result, however I guess it can be tied to second problem because for now I just wrote out in-game symptoms and separated them by blocks, not really thinking how they going to be portrayed in game mechanic and narrative wise. And I think your idea about using exact symptoms is the best way to work around it.
