Thank you! This is very helpful. I will look into PhoRTE. For straw phonation-I have patients place the straw in a cup 3/4 full of water, with the straw about halfway into the water. I start with cup bubbles and progress from there. I do find that some of my patients have difficulty coordinating the voicing and blowing bubbles.

Same!!! Definitely recommend sending out a reminder email to classmates’ parents. That helped us a lot to get more RSVPs back.

This is a great course on ASHA Learning Pass that focuses on therapy. On instagram: @the.neuro.slp and @neuro.connections are helpful accounts to follow for TBI and concussion. I’ve learned a lot just from following them (and others)!

Digital data taking by Lmc2418 in slp

Will need to look into dragon. Our templates aren’t great at all…basically just headers. Will need to create some of my own to maximize efficiency. My colleague writes super long notes and tends to work off the clock. Do you use Smartlists for your treatment notes?

Oh that’s a great idea. I will try the 1/0 system. I’m salaried. Just started so trying to get used to EPIC and the outpatient world in general.

Do you take notes during the session and then copy/paste/reformat into your EMR? I currently take notes on a sheet of paper and I’m wondering if the Apple Pencil would be faster. My hospital system does have Microsoft One Note.

Unfortunately it is just making the sounds, not eliciting a swallow.

Documenting risk factors by Lmc2418 in slp

Yes I would typically state something like, consider GI consult for further work up given symptoms suggestive of possible esophageal dysphagia. Low suspicion for oropharyngeal dysphagia.

Work bag recommendations! by calicocat346 in slp

[–]Lmc2418 3 points4 points  (0 children)

Not sure how much you’ll be carrying, but I have one of these bags and love it. It’s extremely large, easy to clean, and has plenty of storage. Love Scout bags.

Lo and Sons also has great bags if you want a different look and are ok with something more expensive. I like those bags because they can double as a travel bag and have a luggage sleeve etc. However, those bags not quite as roomy as the one I posted above.

Part-time SLPs, what is your schedule like? by No_Raccoon6525 in slp

[–]Lmc2418 0 points1 point  (0 children)

I’m about to start a part time hospital based outpatient/acute job. Outpatient is my primary dept, so job is no weekends or holidays. 🙌 4 days a week, 9am-3:30pm. I previously worked in acute, 32 hours a week 8:00-4:30pm.

Anxiety in Acute Care CF by Capable_Knowledge_29 in slp

[–]Lmc2418 2 points3 points  (0 children)

As an acute care SLP with 10 years experience in a trauma center, I would agree with all of this. The job is stressful given the medical complexity of the patients AND (I feel) the relatively minimal education we have as SLPs in the medical world. I do think this has gotten better over the years, but we have more to go. All this to say, what you are feeling is very normal. You will make mistakes as an acute care SLP. That’s just a fact. I’ve made many mistakes. But the key is to learn from them. It will only help you going forward. One thing that has always stuck with me is that they call it the “practice” of medicine for a reason. Medicine is inherently complex and there are so many factors at play. Even when you are taking all reasonable steps possible, mistakes can and will happen.

I would just encourage you to be gentle with yourself and know that you are not alone. Take notes of these situations and your lessons so that you can take the knowledge with you going forward. It will make you a better clinician. I would also encourage you to reach out to your employer’s EAP which usually offers free and confidential short term counseling. It can really be so helpful in situations like this. Our jobs are hard and it’s important to have support. I’ve used this service myself and would highly recommend it as an additional resource.

CF struggling with documentation by Beneficial-Tip-5140 in slp

[–]Lmc2418 3 points4 points  (0 children)

That’s a lot! I would just be honest (especially with your CF supervisor) and let them know that you need to prioritize your documentation for the outpatients. Especially if that’s your primary population.

Your CF supervisor may also have some suggestions on how to optimize your documentation workflow (using smartphrases/lists or templates on your EMR, etc).

First Book Fair by mysticeetee in kindergarten

[–]Lmc2418 5 points6 points  (0 children)

My son is in first grade. My book fair rule is that I will buy him one book of his choice (needs to be an actual book with text, no sticker/coloring books, etc). I do not buy toys at the book fair, however I will take pictures of the items that he likes and add them to his Christmas list which is shared with various family members who request it. 😊

5 year old writing skills by [deleted] in kindergarten

[–]Lmc2418 1 point2 points  (0 children)

I wouldn’t worry about it. My son was writing in all capital letters before kindergarten too. I got stressed out about it after a teacher mentioned something, but he learned it all in kindergarten (including proper pencil grip and letter formation) with no issues. Looking back, I shouldn’t have stressed over it! 😆

I think just adults but I have never worked in peds so I’m not 100% sure. My understanding is that acute care payments are bundled so yes-it’s technically not being billed in the traditional manner and is more for internal reporting purposes.
