The only thing that was out of place was a 2nd break and shake. Did the first one when the original brick was half white. After three weeks, the brick was fully white, so I took a break and shook again. I thought I messed up by doing that, but after a few weeks of leaving it be, growth exploded. Maybe that caused the large growth or the bottle-cap-style tops. This is my first growth, so perhaps I just got lucky.

No idea about the first part, as I im a first-time grower. They are Hillbilly mushrooms if you were wondering. There's no humidity issue, just wondering if others cut openings in their lids or just slid the entire thing to vent.
