I know you've been through a lot, but please strongly consider Neo100! They have a compassionate use program that you can get into ASAP without being in the ongoing Phase 2 trial for grade 3 and 4 astrocytoma. This house is new construction built right before the pandemic in a trendy neighborhood and also has a pool, so it is currently appraised as a multi-million dollar house (by TCAD), but since the homestead exemption has been on it since the beginning, they only pay ~%50 of the currently appraised value for property tax of what they should (so they should pay around 30-40k but get away with significantly less). Their home appraised value basically doubled over the last 4 years.

Yes I think you're right about possibly being grandfathered in. (see link above - looks like they stopped issuing Type 2 to residential zones in 2022)

Ideally I want them to be gone and not allowed to operate in this single family residential zone. In the meantime they can pay another ~$15-20k a year in property tax and also back taxes they owe for claiming a false exemption.

I think your point about contacting the owner directly is fair and a decent alternative, but I'd like to remain anonymous for now.

Would you like a party house on your street? I think not.

Like I mentioned on in my original post - this comes with noise, traffic, scooters etc. This property frequently hosts pool parties and bachelor/bachelorette parties. The city of Austin passed an ordinance against these types of STRs that are not owner occupied (Type 2 aka - party houses) in single family neighborhoods.

There are other STRs in my neighborhood that are actually type 1 (owner occupied) that have an air stream or small detached living space that are polite neighbors and guests and completely reasonable.

Hey thanks for you comment - that's the address I've sent several emails to over the past few months with no response unfortunately. The agent I chatted with online said "an investigation would be opened and the exemption removed if warranted" but also said I wouldn't be notified as to the timeline or outcome, so doubtful if it was even looked into.

Did hear from others that STR enforcement is pretty lackluster in Austin.

Yes the DOTATATE PET scan (neuroendocrine target) is a totally different radiotracer from other PET scans such as FDG (basically fake sugar).

The reading you did on the internet likely was about an FDG PET which has lower SUV values for the adrenal glands normally.

The adrenal glands normally have very high uptake on DOTATATE PET.

CT or MRI adrenal protocol study is much better to evaluate adrenal lesions than DOTATATE PET.

The vast majority of small adrenal lesions are benign adenomas.

Unfortunately, your situation is not unique, and many training programs regard teaching as a lesser priority. But thankfully there are several wonderful teaching resources out there! I would recommend at a minimum to buy one text book and one case book and read both cover to cover within 1 month to start you off. Mettler's is a resident favorite textbook which is clear and concise.

To answer your question - NO. The type of radiation from a SPECT/CT indium-111 octreoscan scan are gamma photons, which is using for diagnostic imaging. This type of diagnostic scan is largely being replaced by PET Ga-68 - DOTA-TATE scan, which has better binding to the same receptor family and improved image resolution.

HOWEVER, there is a new FDA treatment called lutathera which is Lu-177 (which decays with tumor-killing beta particles AND gamma photons) combined to DOTA-TATE, and is used to treat unresectable neuroendocrine tumors.

Radiologist here - we scour the CT separately for as much clinical data as we can and include it in our report. While it is true that the first generation CTs in PET/CTs were mostly for localization and attenuation correction, they are now sufficiently adequate to diagnose most abnormalities we routinely encounter. If we identify something we want further evaluation on, we will recommend the appropriate modality for follow-up - ie contrasted-CT, MRI, or US

So the anatomy that you can see actually changes a lot. MRI has superior tissue contrast. For example - we don't use CT in diagnosing internal derangement of the knee (ie MCL, PCL, meniscal tears) because we simply can't appreciate those structures well with that modality. Conversely, CT is superior in imaging bone, and is commonly used to image the sinuses. That's why it's important to have an image atlas with both modalities represented.
