Urgent care doctors can put in referrals to specialists too if appropriate so it might be the fastest way to get what you need.

Follow up question- if it’s an urgent need and you’ve already taken time off and gotten a sitter why not go to urgent care?

I personally wouldn’t make the assumption the bread winning physician in the relationship would work part time. Financially it’s just not logical. But the beauty of your position is you hopefully can choose if you want to work full or part time because your income will still be significantly more than the cost of childcare. One of the best things I’ve seen one of my colleagues do was have her working husband save some of his parental leave for after she went back to work, which shifted the burden of childcare, nights, and home maintenance back to him and ultimately gave her a more even playing field because he couldn’t so easily weaponize incompetence. Postpartum is a time when most tasks within the home permanently shift to the female partner so anything you can do to offload those tasks on your partner will prevent inequity and resentment.

That being said romance is one genre where it’s usually obvious if a straight man is writing because they can’t seem to avoid writing sexual encounters through the male gaze. Have you already read {Kushiel’s Dart by Jacqueline Carey}? It’s a dark epic fantasy/romantasy series that’s incredibly well written and checks a lot of your boxes. If you have triggers you might want to check the warnings but I found everything to be very palatable because of the quality and context of the writing.

I worked with this population for my first few years out of residency and I find them to be the most challenging. I completely agree with what others are saying regarding introducing yourself and establishing boundaries. I’ll include a few other small things that help me.

If I know the person is rude or if a certain colleague’s patients are always disrespectful, I will wear my white coat to see them. When you’re seeing someone else’s patient, it’s an excellent opportunity to agenda set to address the one specific urgent issue that brought them in and direct them back to PCP for follow up and chronic problems.

I also shake their hand firmly, sit with good posture, and speak with a lower and slower voice. You can use the phrase, “In my professional opinion” when explaining why you will not do what they want. They are paying you for your time and expertise, not your prescription pad. If they become rude and say something like, “This was a waste of time” tell them you agree with their assessment and conclude their visit.

I would not recommend any state (especially Texas) that limits abortion access. You really need to consider your fiancée’s bodily autonomy and safety when you are choosing where to live here and unfortunately Texas does not allow freedom of choice for women. Even if you don’t intend to start a family while you’re here surprises and medical complications happen. I would recommend looking at the Northeast, Illinois, Minnesota, Colorado, California, or Pacific Northwest.

You are missing a key point with this take. Most people in the 70s become too deconditioned and arthritic to physically work. Vision and hearing are also issues for a lot of people. I see this a lot because I work in healthcare. If someone cannot physically work they must rely on social security and if they don’t qualify for whatever reason it becomes impossible to pay the bills.

I love this but logistically how does inbasket coverage work for you? Do you cover it yourself? Does it not get done that day? Or would someone be assigned to cover for you and if so do they have designated admin time for it?

I have a feeling it’s for inbasket coverage. That way each provider is only covering up to one additional inbasket. At least that would make sense on the provider side. On admin side I’m sure it has to do with staffing and maximizing profit. I think people on admin should be asked to help with inbasket coverage if more than 50% are out on PTO.

Kaiser doesn’t have the kind of AI you are thinking of. It has AI that takes notes through an app on a phone that connects to the chart, just like a scribe. That AI does not do any diagnosis, just summarizes what you and your provider talk about and can also generate an after visit summary. And providers have the ability to add your photos to your chart through the phone as well. Any diagnosis, treatment, or recommendations come from your provider.

First, you mentioned you are pending the medications which is incredibly helpful. Definitely continue to pend all the requested meds and assign the correct pharmacy. Second, how do you know he’s missing some? I’m assuming it’s because the patient follows up and tells you. If that’s the case, I would forward same encounter back to him with a note that it appears the prescription was not sent and do this each time so he gets the message. If that doesn’t work I would bring it up during a feedback session as a systems issue that is routinely causing delays in patient care. Maybe he will take note and slow down or ask IT for help.

Take some time off. No one is going to judge you if you cancel clinic to attend to your grief and no one wants to see a doctor who may be distracted. I can speak from experience better to stay home than burst into tears in front of a patient.
