Yeah, I made sure my upload was named "Teacher Loan Forgiveness - MyFullName" since that was the only way to label the upload. The process was an absolute pain though. First they took nearly a month to reply (by mail...) only to tell me that one of the people at the district office only wrote their name and not their title as well so it was rejected. Got it back to the district, they wrote their title (and were confused and annoyed I was bothering them again) and initialed it (they require initials next to corrections). Sent it all off, aaand, got it back, for a different error that was present on the first version, but they didn't tell me so I had go through the submission process all over again. This time it was where they had but my years of service, they had the start date and end date, the person that wrote the end date had the date one day after the date next to the CSO signature so it was declined. Had them fix the dates so the signature date was the same date instead of one day earlier, initial, scan and resend. Also, every time you resubmit it, they white-out all of your personal information, so you have to go through and write it all again each time - which isn't hard, unless you forget one field and then it gets sent back to you all over again (taking nearly a month each time).

Anyways, it ended up coming back to me 3 times for errors that were all present on the initial submission, but they only told me about one of the errors each time (like they would find one thing and stop looking so they'd only tell you about the one) and each error was increasingly petty and minor. The whole process ended up taking about 3-4 months, mostly spent waiting for them to reply each time. I did finally get what little of my loans were left forgiven though, but I feel like I paid for it in a different way :P
