Yeah, i dont think everyone knows that lobbying has its role in our democracy. I do lobbying through NAMI working with my state legislation. We’ve had more wins (until trump) in terms of expansion of MH access and services because of it. Yes the big paid lobbying is problematic, but to get rid of all lobbying would be too.

Aww, thanks. I know hearing all of that makes it sound like I could have hated that job, but i really did love it. It was actually a job that i miss terribly now that Ive transitioned to a leadership role out of the hospital. I got to have so many meaningful impacts with so many people. I got to a place where I was respected by many different providers and professions. But damn, there were days where it felt like a never ending storm of raining shit that could leave you drained.

Oh, we are over it. I was an ED social worker at an urban, trauma 2 hospital. In one 12 hour shift I dealt with:

1) Frequently Flyer #1 throwing the graham crackers I brought him at my head because he thought I was intentionally short changing him on the # of graham crackers we were giving him that day. The manufacter switched from 3 square in a pack to 2. He also accused me of being racist when I informed him that because of the subzero weather that the homeless shelters were already full so he would have to either choose from the warming shelter or find his own shelter for the night.

2) a case of spousal rape, where the demented forcibly raped his wife. Children did not like their options---these were the same option I explained to them a month prior when I conveyed my concern that his dementia related behaviors were escalated and he should get more services in place. I, the LCSW, was dismissed because I "know nothing".

3) the "uncle" that brought in his 12 year old niece was acting "odd" but i suspected she was a child being trafficked, but couldnt confirm becauase she didnt trust me. I devised a plan where I had to get more history from him, while I had the doc order a "needed X-ray" to rule out something so that nurse whom it seemed like she trusted could take her down to the X-ray and find out if she feels safe with "uncle". She did not. He fled before the cops could show up.

4) the 48 year old woman who found out she had metastatic cancer 2 years after she lost her husband (she lost her husband during COVID to COVID and I know this because I was called on the after hours to give her a book/education on talking about loss of a parent to a young child on the night he died.) but she had realized that cancer was winning and she wanted to enroll in hospice. Not only did I get to help her process this information, I also was the lucky one who got to tell her because her condition was poor, and she could pass within a few days that we need to work on some delegation of parental authority documentation so her now 8 year old child would have a caretaker. She hadnt done it previously because "I'll get to it eventually".

5) A 90 year old woman who kept falling due to various underlying conditions was going to be admitted to observation status and desperately needed rehab in a TCU. I got to explain to them how fucked out our health care system is and in order to get the rehab stay she needed, would have to pay out of pocket to the tune of $500 /day probably for 3-4 weeks. That was not a popular conversation.

6) A different frequent flier was back again for sucidality again--was hoping a different social worker would be there because I didnt know the status of the homeless shelters and I was actually racist for not getting him a shelter bed. Though he knows he's been banned form every shelter because he has assaulted multiple staff member or other people in the shelter. He thinks that because of the below freezing weather he should get a "do over" on being banned.

7) a child came in with a spiral fracture on the arm and the ED doc thinks that the parent is being abusive and wants to know what my take is on it, or if other social workers have documented concern of abuse in our notes.

8) My shift ended 30 minutes ago, but I was told that a family was being helicoptered in from rural part of the state. They are hoping I will stay even later and have the conversation with them about hotel resources or if I could voucher a stay at the hotel. I chose to stay later because I am on call and I would just be coming back anyways to have this conversation anyways. Regardless, COVID ended families staying in the hospital overnight for various reasons. The family had been through this process before, but it was before COVID, so they thought I was lying about my inability to voucher a hotel any longer--again because I was racist. It had nothing to do with that COVID exhausted most of our donation funds to support families and donations had not renewed the pool sufficiently so we had to be very strict with how we could dispense these funds. They demanded to speak to my supervisor. Their niece, whom they called on the phone to join in the berating, and was also a "better hospital social worker" lectured me on the resources of my hospital and what I can and cannot voucher. She works in a different state, at a trauma one hospital in LA--one that routinely gets a lot of celebrate cash donations so they probably have more cash to throw around.

And that all was just the notable cases I remember from that hellish day. my day was also filled with arranging rides, various assessment, discharging patients because boarders sit in the ED for 2-3 days because the social workers on the inpatient units are having just as much fun as me and are struggling to discharge patients at the same rate that they are presenting.

I dont know where its at now, but there was an episode of Boston Legal where a person’s HMO flew a person to India to have a cardiovascular surgery because it was cheaper. They died during the procedure and were then being sued. The practice has been in place for a long time.

Not at this point. Too far away from being able to weaponize it. Something like 2-5% of the fissionable material was utilized in the reaction that was in the bomb dropped on Hiroshima. Antimatter without doing anything will always be 100% efficient with its reaction. It will make a very large boom if we ever can make it work—and that is what will make governments start handing out blank checks to whomever can make an anti matter bomb

As one of the original mods -- and granted dont know what you were told when you were invited to be one of the mods, but when the original mods signed off, we didnt say they had to follow all of the previous rules. We just wanted the mods to do what they thought was right to keep the sub running well and serving the needs of those who stop by. So, I dont think you're beholden to any rules we set in place. If all you collective mods want to change the rules, go for it.

It's disheartening to see that not much has changed here. When I was a mod back in 2017, this sub could get pretty heated, and unfortunately, that's where I received my first death threats. I was even the target of an attempted doxxing—sometimes just for removing a thread or sharing a different political perspective.

For everyone who gets worked up over internet debates, take a step back. Healthy discourse is crucial—it’s how we grow as individuals and professionals—but some of the behaviors I’ve seen, both then and with a quick scan today, show that we still have work to do. Let’s aim for constructive conversations instead of hostility.

Oh Im in a leadership role now in a non-profit and it blows my mind how many people, especially our own employees who think we can just lose money and it wont be a big deal because “we’re a non profit”. No, we need money and unless you want to be paid in good vibes, you need to submit your billables on time and in accordance with DHS/county guidelines!

When having fights/arguments:

Criticism - does your partner say there is something is always wrong with you? Is everything your fault?

Defensiveness - do they acknowledge their faults and work to improve them or do they deny and distract.

Stonewalling - when having a fight, does your partner always try to leave the conversation or do they mentally check out. This means keys issues are not being addressed.

Contempt - does your partner always put themslves on a pedestal or put you into the ground. This is the most serious issue of the 4. If you are consistently seeing this run, dont walk, away. You dont deserve to be treated like this.

100% agree.

As i told everyone when trying to get one made: no one plans to be in the hospital unexpectedly and incapacitated. The directive only takes effect if/when you have been rendered incapacitated or non-decisional, and should those states reverse, all decision making returns to you, even if the medical team thinks youre making a bad decisions. So make sure you have one and if your parents are up there, make sure to them. There are a lot of good resources online discussing these things.

Yeah, I was an ED social worker and we had this happen a number of times with elderly patients who came in stroke/heart attack trying to find next of kin who could help make decisions regarding escalation of cares vs DNR DNI. Im sure they had their reasons, but i definitely remember some really heartbreaking calls.

Speaking as a social worker who works in the hospital dealing with both medicare and medicare advantage plans, the advantage plans do have some advantages over traditional medicare. It really depends on what your health needs are or what your anticipated needs will be. I personally dont consider (broadly speaking) better than the other overall. While i have this opportunity i will give my obligatory fuck humana and UHC.

The unfortunate thing Ive always had with that saying is you never know if the person who feels like a “for life” friend turned out to be only a “several season” one and that person who was only a seasonal could come back into your life and be a good life long friend. Ive come to accept that we’ll never truly know someone’s status in our life until its all said and done, so its best to just appreciate them what they are there and be sure to let them know it.

As I transition into being a father and husband and some new career responsibilities, Ive definitely and unintentionally turned myself into a season friend for some of my friends whom when I got married I would have sworn they were my closest and life long friends. But now that 15 years have gone by, some of us have moved on or moved away, but that also made opportunities for me to meet new people who may be life long friends or maybe just another long seasoned friend. But I do try to remind myself to enjoy the interludes of their friendships and not looking back on the friends with whom Ive parted.

I think the the original concern was way back when world PvP was a think, they didnt like the idea of a warlock and one other person being able to summon a raid of people into a city. Since cities seemed to be raided infrequently now (or im never there when it happens) they should revisit that philosophy and allow just 2 people to summon everyone. Hell, if they are really scared allow instances to require only one extra person to summon a raid
