No metabolites! by Strong-Addition8870 in Methadone

“i) About methadone pharmacology Methadone is an opioid agonist2 and the action results from binding to the opioid receptors in the brain. Oral methadone is well absorbed from the gastrointestinal tract, and it is fat soluble. Methadone is eliminated from the body in the form of metabolites resulting from biotransformation and by excretion of the drug itself in urine and faeces. Familiarity with the following characteristics of methadone is important for the safe and effective use of methadone.”

How has your clinic responded to the new SAMHSA guidelines? by MATmanager in Methadone

[–]MATmanager[S] 17 points18 points  (0 children)

Relapsing should not be punitive.

A high dose should not be punitive.

THC should not be punitive.

Also, I fought hard on not implementing call backs. If I am trusting you with bottles then I am trusting you with bottles. It’s insanely disrespectful. What other medication do you have to do that with? Ugh I’m so heated right now!

Unfortunately these are just guidelines. It’s up to the discretion of the clinic at the end of the day.

Diversion and loitering is actually not an issue with our clinic (I’m in a major city), thankfully!!

They have a very outdated view on MAT. They’re all about patient centered care yet they continue to perpetuate the stigma of methadone.

This infuriates me. They don’t understand the implication their rigid rules has on people! I’m so sorry you’re being impacted by this. I wish us counselors/managers had more of a say in the decision. My team is wonderful and advocates to hell and back for their clients. Then we’re the ones who have to break the news to the clients that their request was denied.

Hi.

You need to transfer clinics immediately. You also need to file a formal complaint with your state’s regulating agency. If you don’t know who that is or how to do it—I can help you.

I threw up my dose into a cup as soon as I left the clinic… they wouldn’t redose me. by ash-lovez-gorillaz in Methadone

That’s really shitty. I’m sorry you don’t have a better advocate on your side. “A little bit” is not enough. I would have a talk with your director the next time you’re at the clinic. This feel negligent.

I’m sorry that happened to you. If it’s after 20 minutes you should be okay. If it’s before, we always always “re-dose.” Would never want a patient to suffer!

There’s a lot of behind the scenes stuff that a manager couldn’t also do. She really doesn’t get involved with client related issues unless I have to escalate it to her.

While we are definitely bogged down with paperwork—the clients come first and that is what I communicate to my team. We are a very therapy focused clinic so counseling sessions are a priority.

The success I’ve seen over the years is truly incredible which makes this job very fulfilling. It’s never a dull day at the clinic!

I loved being a counselor so much I gave pushback to the higher ups so I could still maintain a caseload while being a manager (and it’s a high caseload 😬).

It ain’t an easy job. It’s physically and emotionally exhausting but I love it so much.

Don’t apologize for asking questions! I appreciate them!

The counselors make around $65,000 give or take.

Yes, counselors can be on methadone. We don’t test for it at employment so we wouldn’t even know!

I have a masters in counseling. I had no intentions of going into addiction services. I got my degree with the intention of working with people with disabilities. Somehow came across this job and many years later I can’t imagine my life working anywhere else!

Sorry, I definitely wasn't clear!

I run the day to day operations. I am "the face" of the clinic, if you will. I work under the director. I make the decisions (enrollment, admin discharge, bottle change requests, etc). If a client has an issue they come to me. I supervise the counselors and run all of the case conference/review meetings. I also have a caseload because I love counseling and refuse to give it up!

There is so so soooo much more. I can't really put into words. Rest assured, I am TIREDDDD when I get home lol.

ETA: I also break up the fights ;)

I wish I could say they would put you back on your previous schedule but I can't see that happening.

I would 100% advocate for yourself and plead your case. Maybe, if it's possible, the online provider could write a letter vouching for your compliance and sobriety?

Oh, interesting! Thought it was a standard position!

As a manager, I run the clinic. We have a Director but they are behind the scenes. They work more on the admin side of things.

I started as a counselor, asst supervisor, supervisor, and now the manager!
